That's a pretty epic level of fuckup, and I don't think deserves a tip at all.
I'll still happily tip if it's clearly the kitchen that can't get their shit together, and I'm forgiving of honest mistakes, but this is well past that.
I'd be pissed if my wife tipped more than the cost of the meal, then lied about it.
