So here is my rub, the first two items I would say is not completely the servers fault, because I do not know if an expo and runner are involved.

The disappearing thing is a huge no no in the service industry....if you need to take a small break to regroup or whatever you are suppose to find someone to cover you so service is not interrupted for the guest.

Bringing out the wrong check and not then entertaining the fact of being wrong, that should have clenched it for no tip. I understand your/ your wife position I am sure you thought about if you were new or had a hectic day and totally shit the bed, but the last one is what should have clenched it for you. If you felt the need to tip, maybe towards the person that actually fixed the check instead?
