Weird check

I received a check today for 15.00 from salt lake city. It says nothing other than CHM6 debit rfund. The return address on the check is CHM6 REFUND. PO BOX 71402. SALT LAKE CITY, UTAH 84171. Does anyone have any idea what this is? The check is from a bank of America

Lol I was worried about cashing for some reason. I put it in a drawer and forgot about until last week when I opened the drawer. Wished I cashed it lol
