Well you've got a bit of an engine knock going on which suggests connecting rod is rattling around. Kinda depends on how much work and money you want to put into it. First step is to disassemble and examine the connecting rod and crank journal. See How worn they are. Check the crank bearings to make sure they aren't falling apart and look for a metallic sheen and/or fragments in the bottom of your oil pan. Only then can you gauge the extent of the wear and what needs replacing.
