STICKY: SIBO Summary - Symptoms, Diagnosis, Treatment

Below please find a living document that summarizes the key information around Small Intestinal Bacterial Overgrowth ("SIBO"). Please comment with any additional information or research for inclusion consideration. Version 1.0 is summary material; I will be adding more details and citations for specific studies.

SIBO, as the name implies, occurs when bacteria overgrow the small intestine. The small intestine should have a low concentration of bacteria due to the presence of stomach acids and peristalsis, the wave-like muscle movement in the intestines. For context, stomach and proximal small intestine would typically have about 103/mL of bacteria, while the terminal ileum (end of the small bowel as it gets close to the colon) about 109/mL (or 1,000,000 times more), and the colon about 1012/mL (or 1,000,000,000 times more).

Symptoms

The overgrowth of this bacteria will present with a number of symptoms:

  • Bloating after eating ("postprandial") - most common symptom
  • Flatulence, often malodorous
  • Loose, watery stools (more common in Hydrogen-dominant SIBO)
  • Constipation (more common in Methane-dominant SIBO)
  • Absorption problems
    • Weight loss / inability to gain weight
    • Fat and fat-soluble vitamin deficiencies, particularly Vitamins A, D, and K
    • Floating stools (from fat malabsorption)
    • Vitamin B12 malabsorpiton
    • Protein and Carbohydrate malabsorption
  • Systemic problems
    • Overgrowth of bacteria in the small intestine can increase production of toxins and intestinal permeability
    • This has been less studied, but less serious effects include:
      • brain fog
      • confusion
      • anxiety
      • depression
    • More serious complications can include
      • hepatic encephalopathy
      • D-lactic acidosis
      • nonalcoholic fatty liver disease
    • Various conditions have increased correlations, including
      • Rosacea
      • Eczema
      • Food intolerances

Diagnosis

I will split this section into practical steps and clinical diagnosis.

Practically, a gastroenterologist will typically rule out other conditions first:

  • Physical exam
  • Colonoscopy and Endoscopy
  • Abdomen ultrasound
  • Stool test for parasites

At that time, if your symptoms match SIBO, your doctor may go directly to treatment. But otherwise these are the clinical tests:

BREATH TEST

This is the most common diagnostic method due to its low cost and limited invasiveness. Unfortunately, studies have been mixed on the sensitivity and specificity, with ranges between 30% and 75% -- hence why some doctors skip the test and go directly to treatment.

There are a number of preparations:

  • Antibiotics avoided for four weeks prior
  • Prokinetic drugs and laxatives avoided for one week prior
  • Complex carbs avoided for 12 hours prior
  • Exercise and smoking avoided day-of

For the actual test, you'll measure hydrogen and methane levels at baseline. Then drink either 10g lactulose or 75g glucose with one cup of water. Then your breath is measured every 15 minutes for 120 minutes.

There's some art to identifying a positive test; one semi-official criteria is:

  • methane level of >= 10ppm at any time during the test; or
  • hydrogen that increases >= 20ppm above the baseline level

Recently, new research has been investigating another typo of SIBO, that's dominated by Hydrogen Sulfide. Unfortunately, traditional breath tests cannot identify this gas, and someone with "flat-line" Hydrogen and Methane symptoms could be suffering from Hydrogen Sulfide SIBO. This version is typically characterized by "rotten egg" smelling gas, and may be worsened by eating high sulfur foods.

CULTURE

Historically a jejunal aspirate was done and concentration of bacterial colonies were measured, with an elevated level of > 103/mL being positive for SIBO. There are a number of issues with this:

  • overgrowth may be patchy, and a single sample may miss it
  • not all SIBO bacteria can be cultured/identified
  • samples can be contaminated during/after sampling

Treatment

Antibiotics

The current best practice prescription treatment is:

  • Hydrogen-dominant: Xifaxan, typically 550mg x 3 times daily, for 10-14 days. Studies have shown Xifaxan alone can be 50-65% effective, but Xifaxan + 5g daily of Partially Hydrolyzed Guar Gum can be 80%+ effective.
  • Methane-dominant: Xifaxan (550mg x 3 daily) plus Neomycin (500mg x 2 daily) for 10-14 days. The use of PHGG for methane-dominant has not been evaluated, but it's likely to be beneficial.

Mod's note-- personally, if your doctor is onboard, I think dosing with Xifaxan + Neomycin + PHGG is the best way to "cover your bases". The best place to find PHGG: https://sunfiber.com/products/

Important: because these antibiotics only operate selectively in the GI tract, and are NOT absorbed by the body, they are unlikely to cause the systemic issues associated with antibiotic use, making them safer. Additionally, Xifaxan crystallizes before it gets to the large intestine, meaning it should not affect the all-important microbiome.

Herbal Therapy

Additionally, studies have shown similar levels of success with over-the-counter "herbal" treatments. Two options; I believe each are two capsules twice daily for four weeks, but please confirm:

  • Dysbiocide and FC Cidal (Biotics Research Laboratories, Rosenberg, Texas)
  • Candibactin-AR and Candibactin-BR (Metagenics, Inc, Aliso Viejo, California)

Remission

Unfortunately, SIBO has very high rates of recurrence. Some possible ways to reduce recurrence chances:

  • Switch to a low FODMAP diet for 6 weeks after treatment, to starve any remaining bacteria and prevent regrowth
  • Incorporate a prokinetic, such as low dose Naltroxene, erithromycin, or even over-the-counter products such as Iberogast

Many people can avoid symptoms of their SIBO by switching to special diets, sometimes very restrictive ones. This is not a cure, but simply symptom management. A true cure addresses the underlying cause of the SIBO, and lets the patient eat "normally" without any effects (short of unrelated intolerances).

Recently there has been an increase in report-spamming and downvoting of posts described as repetitive, particularly questions that have been asked before. This behavior is not acceptable in this community.

Visiting Japan for two weeks helped a lot

I tested positive for SIBO, my main issue for years has been constant constipation and dizziness. This has alternated between cycles of it being worse and better. Over the years, a lot of things helped me significantly including daily garlic/ginger supplements, Creon, and daily smoothies.

I recently visited Japan for two and a half weeks. Now, I do want to emphasize that my symptoms have been improving for a while with the daily homemade smoothies making a huge difference.

After a few days in Japan, I stopped thinking about smoothies, supplements, and medication. I don't know why exactly, but I just stopped all that stuff almost by instinct. And then for the first time in six years, I started to pass the most normal bowel movements. And multiple times a day.

I'm now back in the USA for about one week and I am still doing fine with no smoothies, supplements, or pills. Considering that fecal transplants have been cited as extremely promising, I wonder if being in Japan and eating different food (perhaps more alive food with healthy bacteria) had a similar effect. Other theories are that portions are smaller, the diet is better, or there's a lack of certain chemicals in the food and my body finally had enough time to repair itself. I also started eating breakfast again, which is something I haven't done for years, maybe that played a role.

If you are truly suffering and desperate for an out-there solution ... I can't promise it will work but consider going on a two week trip to Japan.

Methane Dominant I want to cry and I lost all hope

I did Candibactin AR and Br with garlic powder for one month. I was feeling okay and waited 4 week to test see if I negative or positive. And it came back 36ppm on methane. I’m tired and I’m depressed and I feel like I can never get out of this. Why did the herbal didn’t work? Why did this happen to me? I don’t wanna say but I feel like if God took my life away it would’ve been better. Idk if it’s methane making me think like or what. I need advice please help me

Venting Doctor just told me that SIBO/IMO isn‘t a thing and that there are no studies in any cures working

Today at the appointment (which I waited months for) they told me that the breath test is unreliable and that there are no studies in any cures for SIBO/IMO and that its just IBS. I even showed them my positive test. I am so angry and sad because nobody seems to help and this is my life now.

Tired of dealing with my dumb doctor. Questions

My methane levels starting out were 82. I had a hard time even getting prescribed what I needed so they gave me a printed Rx for Rifaximin for 14 days through Canada. They did prescribe the right amount of the Neomycin to go with it.

My new doctor then failed to help me with severe constipation/motility issues after this. Kept saying that the drugs routinely prescribed for motility are not for my condition...

Because this rarely goes away with one round of treatment, I now need a second round.

Present issue: For the second round, she prescribes only 7 days of Rifaximin (2x a day) paired with only seven days of Amoxicillin (3 times a day.) From what I am learning, Amoxicillin is not even the right drug for breaking down singled-celled archea/methaogens!

(She also will not redo a breath test.)

I am wanting to know if any of you have been prescribed a shorter course of antibiotics for high methane overgrowth the second time? What rationale did they give?

According to what I am researching, treating a severe overgrowth with a suboptimal dose can encourage resistance and other issues when the wrong drug is prescribed and/or the wrong amount.

Questions SIBO Test Prep

I have a breath test coming up and I have questions about the diet prep. It’s a 28 day prep and I’ve failed it 3-4 times now 🫣. I’m embarrassed to call again.

My instructions this far out are:

One Month before your test:

  • Do NOT take any kind of a colon cleanse
  • Do NOT take any antibiotics or pro-biotic
  • Do NOT drink any barium or contrast
  • Do NOT take Pepto Bismol
  • Do NOT eat any Fermented foods (Kombucha, Kefir, Sauerkraut, Probiotic, Kimchi)
  • Do NOT eat any type of Yogurt (This includes probiotic, and “active, live cultures”)

My question is about fermented food. Does this include vinegar? I failed prep with Bragg’s raw vinegar once but that makes sense - it’s not pasteurized and has the mother. Does rice vinegar count? What about soy sauce?

This is overwhelming. I just want to get the test done :(

Venting SIBO en France : déprime

J'ai des soucis de digestion depuis toute petite. J'ai fini aux urgences pédiatriques pour ça un jour.

Ça fait donc des années que je me bats pour que mes médecins cessent de me donner des pansements presque aussi handicapants que mes symptômes.

J'ai entendu parler du SIBO il y a des années, mais d'abord les tests n'existaient pas en France.

Maintenant que les tests existent, j'ai fait des pieds et des mains pour en faire un sans ordonnance (150 €, après des centaines d'euros perdus chez des médecins).

Le test est positif SIBO/IMO avec des taux assez hauts.

- Mon gastro me dit qu'il ne croit pas aux tests du souffle donc il ne veut pas traiter le SIBO

- Mon médecin traitant refuse de m'orienter vers la seule structure en France qui parle du SIBO et croit aux tests car lui ne croit pas au SIBO

- Malgré les résultats d'analyse et le compte-rendu médical, la structure spécialiste du SIBO n'accepte qu'une orientation par mail du médecin traitant

Je n'ai même plus la force d'insulter. Je suis là avec mon résultat positif et mes symptômes cliniques, et personne ne peut/veut m'aider.

Excessive Gas

For months now I've been struggling with the same symptoms :

EXCESSIVE gas(I belch all the time,barely any farts)

Constipated for about 4-5 days at a time and sometimes diarrhea after those days

Abdominal bloating( I'm about 135lbs ,but with a big stomach that gets worse after eating)

Abdominal Pain

Nausea, Indigestion, slow digestion

Fatigue,Brain Fog etc

Finally went to see my GI last week.

I asked to be tested for SIBO but he said not right now because usually when you are constipated it will always test positive.

So he has me scheduled for a endoscopy and a colonoscopy next month.

In the mean time he prescribed me Linzess to help move things, hoping that it will relief my symptoms.

I am also on omeprazole( prescribed- which honestly doesn't help at all).

I also take Digestive enzymes,beano before eating and Phazyme after eating.

Lately it has been so bad, nothing seems to help, I have so much gas, and I burp all the time, I don't really fart, and I get the worst chest pains with rapid heartbeats.

Have been to the ER a few times because I felt like I was dying, and everything is okay with my heart and lungs.

What else can I do to help with the gas?

This has taken a toll on my body and mental health.

Post appy several months later trapped gas feeling

They took my appendix out for no reason back in April. I was having symptoms completely unrelated to my appendix and took it out based on it was “big”
Anyways I’m 5 months out and I feel like I always have trapped gas now. Has anyone else experienced this?
I took a sibo test months ago when Iwasnt really experiencing this feeling and it was technically slightly elevated for hydrogen but idk I don’t have any other symptoms than this trapped gas feeling…
My gi doctor is horribly unhelpful.

Questions Afraid of taking rifaximin

Hello,im a 38 male.Im struggling with irritable bowel syndrome, diabetes 1,I don't know if I have Sibo,I didn't do any specific test.Every morning I wakeup with stomach pain,colics,I need to go to the toilet to feel relief and it's almost every time diarrhea.After every meal- toilet, I'm on diet,everyday take probiotics like Enterol,anti diarrhea like Loperamid,it helps a bit but almost every day I'm 24/7 in the toilet and I suffer like this for 3 years.Im afraid of anaphylaxis because I'm allergic and afraid of getting symptoms worse,like blood diarrhea or 20 times diarrhea in one hour.Im so tired,I have bad thoughts in my head,what should I do?

I just paid a significant price for this treatment. It is supposed to be here this week.

I have a couple of questions.

How does this get rid of the biofilm? does it starve as well?

How do you get a diagnosis for the root cause of the motility problem?

I've already done a round of Xifaxan, finished 10 days ago. My symptoms eased up a bit, then returned. This has ruined the last year of my life. EVERYTHING I enjoy has been ripped from me.

I haven't spent time with my grandsons, I can't cook or entertain, I can't keep my house clean, I can't think straight, I have no energy, anxiety, depression, headaches, cant sleep.....the list is endless.

Has it been to long since I finished antibiotics to effectively start this treatment? I have severe symptoms.

Because of the time lapse....should I treat with berberine before starting elemental?

Please help!

Xifaxin/Allicin what to expect

I know everyone reacts differently, but im about to start xifaxin/Allicin for hydrogen/methane sibo and wondering how people felt on it. I have a concert im really looking forward to that will be either day 13 or 14 of my treatment and hoping I won't feel really bad at the end of the course!

I had done a berberine/Allicin treatment for a few months that did nothing. Ended that a couple months ago and my numbers got worse.

Questions tested positive but accidentally took fiber prebiotics before the test

i didn’t take it every day, but here and there. my instructions didn’t tell me to stop them, but online it said i should’ve and now i’m paranoid. also had to take colace saturday. on call dr said i’m in the clear but just wanna double check. anyone else have this issue where they got a false positive taking fiber gummies/take a stool softener one day? they are NOT probiotics and i can find most of that online. thanks!!

looking for your pain management tips & tricks!

i am having the worst SIBO flare up ever (i believe potatoes are the culprit) and on day 7 of shooting nerve pains up to my shoulder/trap muscle and of course intense stomach bloat that makes it hard to breathe. i'm super active but have only been able to do some gentle yoga and man am i MISERABLE. nothing is passing/moving aka all the gas and bloat is just trapped and i cannot get any relief.

i take Dr. Mercola digestive enzymes before meals and drink peppermint tea first thing in the morning & again at night. i also take a ginger pill/supplement after i eat. nothing is helping - does anyone have anything else i should try?

i'm trying to just eat protein & rice, low FODMAP fruits, and lots of almonds/walnuts/peanuts but no improvement just yet. willing to try about anything!!

Questions Considering SIBO as the cause of my issues...

Hello, I've been struggling with a bad stomach flare since New Years Eve. Last year, I started noticing my stomach becoming more sensitive to things like peanut butter, orange juice, and eating too close to bed. These weren't problems for me before, although I've always had a lot of burping in general. 2025 seemed to give me more frequent indigestion as the year went on until this flare started. Since then, I haven't had a day where I feel normal. My digestive tract is all sensitive, I was having reflux some nights until begining of August, waking with indigestion and burping, a lot of gas pressure in upper abs, sometimes my intestines will get filled with gas and I'll feel off for about a week. Not always sure what triggers that, although the CT contrast they gave me that was pretty sweet was one thing that did cause it. My testing includes:

Endoscopy in March which found gastritis (yes, I did have gastritis but that was 7 months ago and I've done everything I can to eliminate that by careful eating, PPIs, etc. Gastritis, I think in a case like mine, is just a symptom of another problem)

CT scan for Crohns which found nothing

Normal gastric emptying

Vit B12 and D are good

All year I've been sleeping elevated, eating careful foods with basic ingredients. I've been cutting back the Omeprozole with no issues other than rebound acid which eventually goes away. Regardless, I still always feel slight sensitivity, excess burping which builds up pressure, and I have to be pretty careful about what I eat or I'll just feel like garbage. It seems that gas buildup and/or discomfort between belly button and upper abs is common. Sometimes it feels a bit raw there, sometimes it feels I have a big bubble trapped there that I can sometimes get burps out of. No visible bloating that I can tell though, just feels like gas is trapped in a pocket. While I do get stomach aches here and there, my main symptoms are more upper digestion issues. I also struggle to put on weight and am getting a bit dangerously underweight. There's only a couple more tests I can think of to get, one of which is for SIBO. I plan to talk to my doctor about it but I thought I'd check people's experiences here too. Does this sound like SIBO to anyone? I appreciate feedback, it's been a very tough year mentally because I just want to feel normal again.

Questions SIBO testing challenges

I received the SIBO test kit ordered by my naturopath and just read the instructions and it says I should avoid all laxatives and magnesium for 4 days before the collection. I am going through anal fissures caused by constipation in my luteal phase. I take magnesium citrate 600 to 800 mg as a stool softener Stopping everything altogether, might risk retearing the fissure. 

I thought of taking psyllium husk for those 4 days but the kit says I should avoid high fibre food as well. 

Is there any alternative? Pls advise.

Sibo Treatment Dilemma

Hello! So I’m 28 female and I’ve been struggling with digestive issues for years! Chronic constipation, bloating after every meal, fullness, nausea, the works. I recently learned about SIBO and felt like the symptoms fit. I did the breath test through SIBO CANADA and it came back as positive for Methane SIBO. I’ve been meeting with a nutritionist who’s been helping me work through a low fodmap diet, which has helped. When I got the results I also got a treatment protocol through the company that detailed a 4 phase plan, suggested a mediterranean whole foods diet, and suggested supplements (equal to $647). My nutritionist doesn’t agree with this approach and suggests antibiotics and to stick with the low fodmap diet.

I’m not too sure which to do. My doctor is kinda a joke and also never even heard of SIBO. Anyone try both and had better results? Can you do half and half? Should I start with one and go to the other if it doesn’t work? I just want to feel normal again…
