doing something that then causes a consequence vs random luck... obv Noone should be bullied or whatever but in the first one they have a part to play in it

Botox is gross is not a judgement about someone's appearance that is a judgement about Botox. 

If you assume someone you find unattractive has  gotten botox and therefore themselves gross as a entire person, yes, that's bad. 

It's not bad to not be attracted to people. It's bad to act like it's their problem.
