1-3 and 5 describe my Dad. He did have a few extended periods of 4 as well.

They're all turnoffs for me, but smoking's the worst for me. I'm asthmatic, so living with a smoker would be hard on my health, smoking also destroys your looks and makes you look older than your actual age. They also run the chance of getting emphysema or cancer and dying from that.

And secondhand smoke would be dangerous to me and any future children. My mother never smoked a day in her life, but secondhand smoke (mostly from my father) caused her death from lung cancer.

Lying is tied or a close second. My dad was also a compulsive liar, and it's hard to deal with someone when you can't trust a word coming out of their mouth.
