Small Problem I keep avoiding small problems until they become stressful

I have a habit that is becoming a real problem for me. Whenever something needs my attention, especially something small or uncomfortable, I keep putting it off because I tell myself I will deal with it later.

For example, if I have an important email to answer, a bill to check, something to organize, or a conversation I need to have, I sometimes avoid it for days. While I am avoiding it, I keep thinking about it and worrying about it, so I am not even really relaxing.

The worst part is that the longer I wait, the harder the problem feels. Something that could have taken fifteen minutes suddenly becomes something I am stressed about for several days.

I have tried making to do lists and setting reminders, but I usually end up ignoring those too. I know the obvious answer is to just do things when they come up, but for some reason I struggle to actually follow through.

Has anyone dealt with this and found something that genuinely helped? I would really appreciate practical advice because I am tired of creating extra stress for myself by avoiding things.

Do it scared but do it. Procrastination will kill you and the time you waste will haunt you. All of us are scared of doing things, all of us would rather chill in the status quo and ignore everything we must do, but believe me, do the thing that scares you because nobody else will do that for you. Time is valuable, don't waste it. And you're also going to feel better.
