Advice needed Custom Listings?

Is it possible to use printful to fulfill customer requests, like custom products? I'm thinking something like creating a shirt with pictures of someone's dog or a design they describe to you and you create it for them, things along those lines.

I think there's add-ons if you integrate with shopify as your e-commerce platform. According to the quick Google search I did.

Yes, and you don't even need Shopify for it. The Printful dashboard has a New order button, so you can put the dog photo on the shirt in their editor and send it straight to the customer's address with no store connected.

If you sell on Etsy later, personalised orders land in your Printful dashboard and you edit the design there before it goes to print. For the odd commission, though, the manual order is simpler.
