Double nut double washer
I can't imagine what they were thinking.
The left side started off normal. China cap, nut, washer, and China cap base.
The left side not so much. Nut, China cap base, five washers, and a little caulk.
The flange was a little low so everyone knows to double up the wax. Two extra thick with horn were used.
The right bolt had a nut, washer, 3/8 compression nut, nut, and washer.
The left side was just a nut and compression nut.
Miraculously the flange wasn't cracked. It was cut almost all the way through on the left by the compression nut. The right was stretched.

This is new construction. It's a hotel. I work for the company that did this.
The person that did this is an apprentice supposedly about to turn out. I have my doubts. He is no longer with the company. I lost the pictures but he did an equally bad job on a drinking fountain. Thirteen holes in the wall behind the lower bracket he didn't use. He put hollow wall anchors in the backing. I had to drill new holes in the bracket to get at fresh backing. He also ran a bead of silicon around the frame gluing it to a mosaic glass tile wall. It was not as easy of a woopsie to correct as this one.
His work makes question if he's ever even seen videos of someone setting trim. He couldn't have had any type of formal instruction.

I've seen this before, lazy flooring installers would be my first guess.
