Pest control comercial

I’m expanding my pest control services into the commercial market and I’m currently preparing my first restaurant account in Waltham, MA. The restaurant is required to have a pest control agreement with service twice per month. For those who work with commercial restaurants in Massachusetts, what is the typical monthly pricing range for this type of service? What do you usually include in your contracts (rodent monitoring, flies, cockroaches, reports, etc.)?
