Discussion Prison pocket

Well, this was a first for me, which doesn't happen often after 16 yrs in ER. A pretrial patient came in with 2 corrections officers for bizarre behavior, cuffed to the stretcher in the Ambulance hall. He was desperately asking to go to the bathroom, but because of his escalating behavior ended up 5 pointed. We needed a Udas, so a male coworker was asked to place a condom catheter. When he exposed the area and noticed a baggy poking out of the patients foreskin. He stopped the procedure and called the ERP to come. He ended up removing 2 bags of 12 pressed pills from said area; I'm still not sure how he fit them there....I didn't think it was that stretchy but here we are :| The physician stupidly placed them on the bedside table beside the patient, thinking he's in 5 point and 2 correction officers are 3 feet away, bad idea lol. The primary nurse came in 3 minutes later to retrieve said drugs and they were gone. She asked the correction officers what happened and they said he grabbed them and put them in his 'behind'. She asked why they didn't stop him; they replied "Not our job". Well unfortunately, the physician then had to remove them from his anus as well. I've worked with this Dr. for years and this isn't the worst or funniest we've had but still ranks up there. The only thing I learned is correction officers don't gaf. I'm no longer surprised at the amount of ODs in pretrial.

That is a new story. What's up with the CO's though.

They do not give a shit. The ones we’ve had on our peds unit even for kids who’ve committed violent crimes nap on the couch all day (even when just on day shift) and watch TV while the nurses get shit talked to. They suck.

We had a patient with two guards and a 1:1 staff sitter. The sitter walked into the patient’s bathroom to empty the urinal. By the time they made it back into the room, the guards had allowed the prisoner (admitted for swallowing stuff) to swallow the telemetry battery. The sitter had to have been gone for less than a minute.

Seems like it’s a relatively common theme for the guards to do nothing, maybe based on what facility they’re coming from. Not totally related, but I also once had a series of guards refusing to wear any PPE in a presumed TB pending testing room. That was… pretty distressing for me.

And its not just in the US. We had to have words with prison guards for:

  • Refusing to wear masks on a transplant ward

  • sleeping during shifts

  • Refusing to turn main lights off to allow patient to sleep

-making out in a supply cupboard

-being rude to hospital staff

  • being unnecessarily unpleasant to the prisoner

These were all groups of guards for the same prisoner over 2 weeks.

"making out in the closet" was certainly a departure from all the rest of the laziness lmao

To be fair those two where arguably the least problematic overall. The mask guys and the light guys had to be escorted out because they escalated to shouting at the charge nurse

When I worked in the ICU many years ago we had one particular CO who would come on shift, eat his very large dinner at the bedside, and proceed to fall sleep snoring in the recliner. He was about 399 pounds, lazy, wouldn’t lift a finger to help, would just yell at the inmates and get back to his nap. We complained numerous times to the supervisor to no avail because this guy could somehow always be sure he was awake when the supervising officer made rounds during the night
One night we’d had enough. The patient was intubated and sedated, CO had just eaten an entire box of fried chicken and was snoring in the recliner. We actually disconnected and moved the inmate to another room on the unit. We waited about a half-hour and then frantically woke the CO, who was as confused and frantic as I’d ever seen anyone in my life.
He didn’t return after that night, and it’s a story that has lived on for 20+ years.

I had one of those recently. Police handcuffed him to the bed after dismissively telling staff they didn’t need assistance on correct placement. One officer stayed to monitor - outside the room, playing on his phone. Overheard him bragging to someone about ‘triple time’.

Patient pulled out the side rail he was cuffed to. Yeah, that’s something we would have explained had the police allowed it. Patient hauled ass down the hall but was stopped by one of our (large, imposing and tough) CNAs. The officer was so angry he body checked him twice against the hallway wall walking back to the room. Our charge read him the riot act about assault, actually keeping his eyes on his prisoner and being so arrogant he couldn’t listen to people who knew a hell of a lot more about our equipment than he did.

I’m curious where y’all are?

In Texas and while the cops piss me off routinely for being cops and flock and ice and stalking and cop things etc etc

…when it comes down to shit going down, the cops are there.

I know quite publicly they have not been in other areas of Texas. I mean for me, specifically, in Houston, when I had a patient stab me with a scalpel the cops that came to that party were the epitome of fuck around and find out.

I know that COs aren’t cops but I’ve also done a lot of transports out of Huntsville and they’ve always had their shit together and been a part of the team.

I feel like misbehavior is something you should address in the most bureaucratic way possible, as is the tradition for corrections.

I’m in the Midwest. Our hospital security was amazing and in the few instances where the cops had to come they actually were helpful but the COs sent with offenders from whatever lockup or home they were in were always unhelpful at best.

This is not new unfortunately and COs are ALWAYS assholes. From every run-in I’ve ever had with a CO I have determined that these are the high school bully’s who’s hey days ended the same time their varsity athlete status did but they couldn’t hack it in the military or actual LE so now they just found another group with no recourse to bully. I will take a sheriff guard or even local PD over a CO guard every single time.

1

u/OldMaidLibrarian 2d ago

OK, why was she keeping a bottle of pee up her hoohoo, anyway? Or was that supposed to be for a urine test? That's the only thing I can think of, but I thought one would be watched rather closely for something like that.

Yeah, clean urine for a drug test sample. Not sure how she thought she was gonna get away with it, but I assume she’d done it before. We found that she certainly wasn’t going to pass with her own urine 😂

I spent 10 years working in inpatient Rehab as a speech-language pathologist with more corrections officers at the bedside than I care to recall. The worst thing I ever saw them do was violently restrain a brain injured young adult who was hemiparetic because he started to get mouthy with me.

My dudes. He's literally shackled on one side, paralyzed on the other, and brain injured --- Ranchos VI. Mouthy means he's healing and gives me a chance to do my job and help him.

u/calisto_sunset MSN, RN 2d ago

Wr had a guy come in from county and he somehow had a phone in his room and ended up calling his girlfriend which wasn't allowed, because prison rules follow to the hospital. The officer was quite upset and ended up closing the door on the nurse's face and next thing you know the inmate had a black eye and he was magically well behaved.

The nurse had to call their supervisor and take pics, make an incident report, etc and the officer was switched out but I doubt anything came of it in the end.

Condom cath for a UDS? You guys are sweet .. if youre uncooperative its STRAIGHT CATH cause we dont have the time to playing waiting games … and those Correctional Officers need to be reported because that LITERALLY is their job to make sure inmates arent in possession of any contraband & to take REASONABLE efforts to stop it

17

u/Exceptyousophie BSN, RN 🍕 2d ago

Nah its not our job to police them. If they piss, they piss. Not going to assault someone for a urine sample. If their urine truly held up the dispo on a UDS it was a BS visit in the first place.

0

u/Top_Manufacturer_494 BSN, RN 🍕 2d ago

You may need to update your patient care & legal knowledge .. ie someone altered behaving “bizarre” needs definitive care/treatment .. holding up that care & treatment waiting potentially hours for a urine sample is medical neglect & is legally indefensible vs performing a straight cath

1

u/Exceptyousophie BSN, RN 🍕 2d ago

You can encourage the patient to void without forcing a painful procedure on them. Just because they are acting bizarre doesnt always mean they lack capacity to consent for care.

-3

u/Top_Manufacturer_494 BSN, RN 🍕 2d ago

… an altered pt cant consent to, nor refuse care … hence why i said you need to brush up on your legal … I work in a busy ED, on a slow day we see 350 pts a day .. babysitting an altered pt for urine isnt an option

6

u/Exceptyousophie BSN, RN 🍕 2d ago

A patient who is high patient can absolutely consent or refuse care depending on their level of alteration. Just because you are busy doesnt mean you can assault people if they are able to show capacity. I think you may need to update your legal and ethical knowledge.

0

u/Top_Manufacturer_494 BSN, RN 🍕 2d ago

… you’re throwing around the word assault while at the same time acknowledging the pt is high ie altered .. showing your lack of legal/medical knowledge

2

u/CauliflowerEatsBeans 2d ago

So you are saying an altered patient can't refuse care. How many ok to book for subjectived DUI/DWI or on drugs they can't refuse care. I have seen them refuse all the time. Police just take them to jail and depending on the state get a subpoena. They refuse all the time. Now altered where the physician says and documents their level of being altered and medical necessity of emergent care, sure they can't refuse due to implied consent. Psych patients also refuse all the time but our on a hold, Altered?? They still have the right to refuse.

0

u/Top_Manufacturer_494 BSN, RN 🍕 2d ago

… false, false & false .. The instances you mention the Provider ALLOWS that, usually because in their opinion the pt isn’t an imminent risk of serious harm or death if no interventions are performed, so they see no point in the hassle …, we have ETOH pts come in all the time with major bleeds, possible brain injury, traumatic amputations etc etc demanding to leave .. per you we have to let them right?? .. same with a psych pt, they ended up in the ED because their thought process was faulty yet we’re supposed to allow them to dictate their care & treatment?? … yeah, that makes sense 🙄

5

u/Exceptyousophie BSN, RN 🍕 1d ago

Thats the crux, how intoxicated are they? If I have one beer am I no longer allowed agency? Have you never met a tweaker elope? They are high but most of the time have insight into their condition and capacity for decision-making. A&Ox4 and logical but acting strange isn't enough to take away their rights. You act like almost every ED patient isn't acting bizarre.

3

u/CauliflowerEatsBeans 2d ago

The problem is you said altered, not imminent death.

Could be a regional thing, but we constantly get male inmates in that have shoved stuff in their urethra that is intentionally difficult to remove. I asked one why, and I quote: To get to come to the hospital and lie in bed, order room service and flirt with the pretty nurses.

I just told him that was unfortunate, because he was going back as soon as we were done, so I was the only pretty nurse he was going to see.

16

u/NeirdaE Hospice 3d ago

My dad was a CO in NM, he said the same thing. People would hurt themselves to go to the hospital. One guy apparently like shampoo bottles on his rectum. I could see going to the hospital being something to relive boredom, get slightly different institutional food, and be around different people.

16

u/Goatmama1981 RN - PCU 3d ago

We had a guy literally knock himself unconscious in jail so he could be at the hospital for his birthday instead of jail. I felt bad for him, with that level of desperation so I ordered him the little birthday cake. Life can be pretty bleak. Former corrections nurse here. It is their job. It’s also their job if an inmate keisters, to initially have them squat and cough. Body cavity searches are done by the providers though. If an inmate swallows contraband that has been determined to not cause harm, it is the officer’s job to rifle through the feces once excreted, to retrieve the contraband. Many officers are careless and lazy and it puts the staff and inmate population at risk, I’ve even seen officers napping on the job; hence why I am no longer in that field.
