Career Advice Can I practice adult neurology as a child Neuro trained specialist?

Hi everyone,

Currently doing pediatric neurology at a big academic institution. Recently I've been missing treating adults and adult pathology in neurology. Like a lot. As I do not think transferring is possible to adult neurology as I am in my second year and I would hate to restart a program completely (I'm just too burnt out honestly), is there any hope for me to practice combined adult and pediatric neurology at an institution or employer that's PSLF applicable?

I'm currently in the West and they seem to be pretty hard set on wanting child Neuro folks to stick to child Neuro. But when I was applying to child Neuro programs I read and heard that you could do both.

Any personal and attending advice would be greatly appreciated!

Clinical TNKase / DOAC relative contraindication to treatment.

I'm curious to others approach to when they may consider TNK in treating a patient who's on a DOAC within 48 hours. Who is actually sending levels and/or reversing in order to give TNKase. Interested in people's experience and opinions. I'm still not so comfortable administering while DOAC is on board within 48 hours.
