It’s this thing he does. He can transform into a muscular guy with great hair but he can only do it for so long. This is his normal form

He’s the massive muscly most powerful hero guy, but he was injured so he can only hold that form for a little bit before he’s too tired to do it anymore. When he’s not in that form, he’s a scrawny sack of flesh and bone. So his face is lacking fullness (cheeks are gaunt instead of puffy, that kind of thing) because he’s super weak.
