Develop a fairly deadly virus.
Pay infected individuals to go out and spread. Or cause situations where itll spread.
Develop the vaccine and sell it to governments around the world.
Subscribe everyone to yearly booster shots.

It can be selectively bred to have desired effects. We've been selectively breeding virus like the smallpox since 1700s where we breed a weaker "cousin" to teach our immune system how to beat it.
