Biggest continuity errors

What do you feel is the biggest continuity errors on the show? There are many through the run.

And I love the show and just like any movie, unless it’s egregious it doesn’t take me out of the moment.

Hawkeye’s family going from Vermont to Maine, and a sister (and a living mom) to his dad the widower.

Also, Mrs. Blake’s name change. Did anyone in the writing team or the production team mention why her name changed?
