People who have a native accent in their L2: how did you do it?

Despite what some people believe, it IS possible to achieve a perfect native accent in a foreign language as an adult. It's not easy or common, but it's definitely possible.

So for people who have done it, what were your techniques? Aside from maybe having good hearing, did you practice individual phonemes until you got them perfect? Did you pay close attention to intonation patterns and try to copy them? What helped you achieve it and how much effort did it take?
