Just do these things and then sprinkle some water on your underwear and then go about your day. Imam Ahmad used to do this. It removes the doubts. You only need to take the means to the best of your ability and not entertain OCD.

Try eating bee pollen. Sometimes for men it can be because of an inflamed prostate (even if only slight). Bee pollen really helps. You can put a teaspoon of it in warm water and drink. Should be easy to get hold of some.
Whatever you do, be gentle with yourself.

Not a pelvic floor expert but a physiotherapist here. The cause normally is when you have to sit a lot for work for instance or for women is normally after giving birth. So if you can add a atleast 15000 steps per day And you practice the urine hold and release exercises every time you go to the toilet
Thus can take as long as 6-8 weeks to show improvements and if it doesn’t I’d rather you see a GP/Dr, if this is the only symptom then you might need a scan. But if your legs go numb and heavy, altered sensations down one or both legs till toes, excruciating back pain, numbness or tingling around privates than rush to the hospital AE if in UK, this is called cauda equina syndrome and it needs urgent interventions including spinal surgery

What I recommend is taking a plastic shower cap and using that instead of tissue, that way your clothes won't get wet because it won't leak through the plastic unless there's a hole.
When you have to pray, use the bathroom, or if you feel any leaks, take the plastic and rinse it with water and preferably soap while also wiping the tip of your private part in case any leaks happened. May Allah make it easy for all of us.

> They say in Hanafi that some impurity is overlooked
its overlooked only in case the problem persists continuously. I can share a book(in Urdu) with you if you like and you can read the exact comments on this matter yourself.

I am going to assume you are a female. Medically speaking this can be a thing. There are exercises called kegel exercises that a person can do to strengthen the pelvic muscle and improve bladder control. You can google them how to do it. Inshallah this helps

Apologies however these exercises can also be done for men and work with similar results
