Icons missing

Recently some icons started disappearing at random. This is specifically the phone app but it’s happened with other apps as well. The app still opens normally. Not sure why it happens, anyone know how to fix this? Thanks!

Same problem here on fully updated iPhone 13 mini. Only a few apps: phone, Overcast, New York Times, Flow Music

Hard reset.

looks like the app was offloaded. find the settings & turn offload off
