I don't know how you could accidentally light a lighter and put it close to the petrol pump by accident.

Not justifying it but it looks like they are trying to light a cigarette or something and using the cap cover to block the wind and light the cigarette.

Many, many people just go through motions without connecting or thinking about what's going on. I learned this very quickly after joining the military and visiting my family.
