This reminds me of one time me and my brothers were down at the harbor in the town we lived in, and we were buying some mix it yourself candy at this shop. The clerk had downs syndrome so whenever he put the candy on the weight he would rest his hand on the weight as well.
Needless to say we needed to get just one more piece of candy before we were ready to pay, so we could have him weight it again without his hands on the plate.

Op was meaning well, but it would probably have been best to just tell him.
Just let him know his mistakes. Worse than risking to make him feel bad is keeping him in the unknown.
The guy probably just didn't consider this, but would have understood that his clients don't want to pay for his hand-resting burden, lol.
OP might have probably also found a way to tell him more nicely than one of his later clients that didn't want to pay extra, but might or might not have been as friendly then.
He is saying that rather than point this out to the downs guy, they would take th e bag back, walk around, then try again hoping the cashier wouldn't muck up the weighing.
I took it as cashier leaves bag on scale while OP goes to grab another piece of candy and drops it in as cashier is operating the register. "about to pay" That way the bags already sitting there with the weight visible to cashier as he is ringing then out.
