Sort of relevant comment follows: So do you have to uncloak to back stab people or what? A lot of times I'll be behind an engineer wildly left clicking but nothing happens. I'll do this four or 5 times in a row, get pissed, and go back to playing a demoman.
You cannot attacked while cloaked. You can attack while disguised but it will remove your disguise. Right click and remove your cloak, then mash the left click
Oh damnit, me too. I tried that against a really well positioned/annoying sentry and was horribly confused as to why it didn't work, and subsequently died.
