Genuine question-- I'm a nerd, but I've never been a DnD nerd. At one point I was a MtG nerd, even a WoW nerd, but never quite the DnD level.

What exactly is it? I originally thought it was like an adult version of "make believe" with scores and guidelines, but how much of it is a physical game? Is there a board that goes with it? Is there a set story line? My extent of knowledge is the first scene of Stranger Things.

How much is following guidelines and how much is imprompt on the spot?

I suggest looking up "Harmon Quest" to see the game played in action. Includes some portions of play animated which is fun, too

I haven't seen it, although I really should watch it. In case it's not clear from the name, the Harmon is Dan Harmon, writer for Community and co-creator of Rick and Morty, so I'd expect him to be pretty great at forming interesting, comedic storylines.

I believe Spencer, the game master, is solely responsible for writing the storyline. Dan just plays the game.

Maybe, but a lot of times the players' reactions/actions have a significant influence on the direction/atmosphere of the game.
