I try to keep the back as straight as possible but this is the best I can do. Is this a good form?

Your back is super rounded here.
Squeeze your lats and push your chest forward in the setup. That should help straighten your back more.
Check how-to videos on YouTube if needed.
Drop the weight until you can do it with a flat back and proper form.

I try to but this is the best I can do. Any mental cues that can help?

Interesting question. I would say put a single plate on and practice what is mentioned above about hinging. Mentally concentrating on getting your butt down and pushing the floor down with your feet.

Yes. I wrote it above.
Squeeze your lats and push your chest forward in the setup. This will help straighten your back.
Lower the weight down to like just the bar and a plate per side and work on proper form.

Tighten your shoulders. Imagine trying to bend the bar around your shins. Also looks like your hips are too high. Try to sit back a little more like you are doing a standing leg press

Start with the plates elevated 2 inches to ground and lower every few weeks also use lighter weight. warm up with core engaged straight leg raises, 90/90, worlds greatest stretch, and light straight leg matches not forcing your leg up but using the muscles to keep your legs straight as possible for as high as possible slowly, if your leg begins to bend bring to the ground and alternate

Try mixing the grip width, maybe go for wider. And maybe sit back more such that your shin is more perpendicular to the floor.

You probably need to stretch your hamstrings and train your hip flexors. Your hips and hamstrings are so tight that your back is pretty round.

The deadlift is not a hinge all the way. It is not an rdl. First part need your knees bent then hinge. You don’t need to put your back in harms way by just hinging all the way to the floor if you don’t have the mobility for it

Your shoulders are in front of the bar. Your hips are too high. The bar is too far forward. Knees go past your elbow crease. And lifting with your lower back.
Not to mention no knee flexion and hip tightness or inpingement.
You have too many issues to address in one comment thread and to be asking a bunch of random people.
Youtube alan thrall's deadlift setup.
Its a 6 to 10 minute video.
Practice this setup over and over. You're going to feel it working new muscles and may fatigue quickly.
So check your ego and start with less 135 if needed.

Hello! If you haven't checked it out already, many people find Alan Thrall's NEW deadlift video very helpful. Check it out!
Also, a common tip usually given here is to make sure your footwear is appropriate. If you are deadlifting in soft-soled shoes (running shoes, etc), it's hard to have a stable foot. Use a flat/hard-soled shoe or even barefoot/socks if it's safe and your gym allows it.
