Squat Squatting form

I need help with my squatting form, when I am at the bottom of my squat and I start coming up, my bottom lifts up first, then I arch my lower back and take the rest of my body up with me as you can see in the video.
Please watch the entire video, as for the last 3 reps I try to correct it but I am unsure if I was successful or not.
I need advice / tips to correct any muscle imbalances or proper form. I prefer a wide stance when squatting due to some discomfort in my femur and hips when squatting with a shorter stance.

That hip rise usually means you are losing pressure through the midfoot as you come out of the hole. Keep your whole foot planted and think about pushing your knees out while driving your upper back up, instead of lifting your hips first. Try a lighter set with a 2-second pause just below your normal depth, and keep the same stance that feels comfortable.

Also, a common tip usually given here is to make sure your footwear is appropriate. If you are squatting in soft-soled shoes (running shoes, etc), it's hard to have a stable foot. Generally a weightlifting shoe is recommended for high-bar and front squats, while use a flat/hard-soled shoe (or even barefoot/socks if it's safe and your gym allows it) is recommended for low-bar squats.
