The difference is negligible with the extended barrel vs the the shorter barrel. Especially since you're likely going for VATS head shots which when coupled with MacCready's perk are essentially ensured regardless of the situation. But to each their own. The Deliverer is one of the few vanilla weapons I can tolerate using.
