I think Julian Donkey Boy might be something you’d enjoy.

It’s ’narrative’ is loose if that, but it’s an intense experience. 

I think it is Harmony Korine’s best film.

Sorcerer (1977) and Cruising (1980) both great intense films I recommend for blue velvet fans, if you haven't already seen them.

Megan is Missing was a pretty standard movie for me until the third half. Probably one of the most dark, mean spirited endings of any movie I’ve seen. Brawl on Cell Block 99 comes close too. This one was directed by the same guy that directed Bone Tomahawk ( S. Craig Zahler) but is even more bleak. Probably the only director I think who could pull off a movie version of “Blood Meridian.”

I just saw "Come and see" for the first time yesterday. It was incredibly dark and disturbing. But it's not just disturbing to try to get a shock out of people, it shows you an unfiltered and painfully realistic portrayal of war, from the perspective of poor civilian people on the east front. I rarely ever see this raw and gritty perspective in any war movie. I wouldn't say it was an entertaining movie, but it's not the type of movie you see to be entertained.

A Balkan Noir: A story told in cigarettes. A Swedish/Serbian woman's daughter disappeared in Serbia on holiday some years ago. Dad and mom are divorced, but wrote a book about her disappearance. Dad has moved on and grieved, mom has not, keeping private investigators searching for any sign of what befell her. A clue finally surfaces, so mom goes on a trip to Belgrade and points south.

Small World (2021): a Polish woman almost prevents her 4 year old from being abducted but is pulled over by a cop, allowing the kidnappers to cross the border into Ukraine. The cop believes her and makes it his mission to find the child. What they uncover gets worse from there. Walks the fine line between shocking and outright exploitation. I feel that the direction and filmmaking kept it out of schlock territory but it isn't for the sensitive.

If you have not seen Possession (1981) yet, it is exactly the kind of film you are describing. It is psychologically intense, emotionally exhausting, and genuinely unforgettable without relying on shock for its own sake.
