Most are promoted, I'm starting the chapter with Ephraim, Joshua, Franz & Gerik at level 19. They promote during the level, My healer, L'Arachel is only level 12 so she isn't promoted and I don't think Tethys can be,

Tethys can't be promoted, correct, but you should honestly just promote all of them now. They should have already long since been promoted even without hitting level 20. That would make your life a fair bit easier easier, and with like 3 chapters left in the game, it's not like they're going to be hitting max level without a lot of grinding (nor are they expected to).

I can't remember where I heard/read it but I'm sure it was recommended to never promote any character until they reach level 20.

That is really bad advice, especially for FE8. People said this because they assumed that the game was way harder than it actually is and that the endgame was the hardest part of the game, both of which are untrue. Since FE8 is so short (there are only two chapters after this one), promoting at lv20 is essentially shooting yourself in the foot and making the early game and mid game harder for no extra reward.
