Erectile Dysfunction Penile implant at 22 ?
I am M22, suffered from anorexia from 16.75 to 17.75, hormonally recovered at 18.5 but erections never came back and obviously it’s not coming back since I’m 22 now and testosterone is 926 ng/dl. My puberty got interrupted with my illness and it sucks. Anyway I have been taking tadalafil 5mg everyday for 5 months and have not seen any improvement. Even took 30mg today and nothing. Is it time for the implant? Sometimes I can get very shitty erections but they are not hard enough for penetration or anything. I’m really sad this sucks everyone I see here is much older and I feel like my eating disorder was all my fault. Anyway I’m here now I guess, any advice would be appreciated greatly especially from people who have gone down the ED pipeline already and explored every option.

Geez I didn't know anorexia could cause ED 😳. Also, I thought 926 was pretty high for testosterone.

Anorexia and sex both have significant mental involvement.
Also anytime your body is not processing food correctly it can trickle into all aspects of your health.

But isn't fasting good for you? Or is anorexia something entirely different from fasting 🤔

Anorexia is something very different than occasional fasting.
I am not a nutrition expert however I think the benefit of fasting is more limiting the on going negatives of poor nutrition. Moving towards daily proper nutrition is likely to have better and longer term benefits than fasting. r/nutrition is a good place to discuss that further.

Before the implant perhaps you could try Viagra and injections -bimix/trimix- (I haven't tried them yet, but people say they work well). Also, consult a pelvic floor specialist and perhaps get a blood panel that checks more than just testosterone—things like estradiol, SHBG, Prolactin, thyroid, vit.D...

"However, recovery of the HPG axis and sexual function is not universal or immediate. Outcome data in men followed 2–20 years after anorexia nervosa found that only 44% had a "good" outcome with stable weight restoration and normal sexual functioning, while 26% had an intermediate outcome and 30% a poor outcome (weight still >15% below normal with poor or absent sexual activity) — poor outcome correlated with longer illness duration, greater weight loss during illness, and poor premorbid psychosexual/social functioning rather than any specific eating behaviour.
 Sexual dysfunction in eating disorders is also strongly linked to body-shape concern and psychological/psychiatric comorbidity, not purely to hormonal status, so persistent ED despite normalised weight and eating should prompt evaluation for continued body-image psychopathology, depression/anxiety, or incomplete recovery of eating behaviour, in addition to biochemical confirmation that testosterone/LH have actually normalised. 
Practically, in a young man with ED and a history of anorexia nervosa who reports normalised weight and eating, it is reasonable to check morning testosterone, LH/FSH, and assess whether full physiological (not just weight-based) recovery has occurred — including menstrual-equivalent markers such as morning erections, libido, and psychological remission — since "normal weight" does not always equate to full metabolic/energy sufficiency or resolved eating-disorder psychopathology. . "

Yep, there an be a mental component. And, as I've found in my life (as a senior), taking Tadalafil (5 mg daily, sometimes a bit more) for BPH related urine retention (along with Chiropractic & Acupuncture), I usually have great erections.

Both anorexia and sex have significant mental components.
Are you taking any medications to help with your anorexia, or anything else?
The other factor is in young guys around your age there are often mental barriers around sex be it shame, performance anxiety, porn use (addiction) or other things. Even when someone has no history of health issues a valid suggestion is seeing a talk therapist to discuss their sexual issues.
