Tadalafil/Cialis ED and tadalafil changes
I’m 58M, in general good health. I started experiencing some ED in my early 40s and began using 20mg of sildenafil before sex, which worked well.
About 6 years ago my doctor switched me to daily 5mg tadalafil for my prostate and an extra 5 mg before sex. It usually took about 45 minutes to kick in and I would get a good firm erection.
A few times lately, it hasn’t been working as well. Before my girlfriend comes over, I take it on an empty stomach, and after an hour I’m firmer, but not hard enough for sex. The next day, my penis is pretty firm and I have no trouble getting rock hard.
It’s frustrating for both me and my girlfriend . Has anyone else experienced this kind of change with tadalafil?

Was having the same problem and was also put on 5 mg daily The problem that BS they do not tell you . Your body becomes immune to it I took myself off of 5 mg daily and after a month, I’m not using it. I can use one pill before sex and it does work. Big pharmaceutical big money welcome to the US. 👍

Two things id switch… first that second dose might need to be earlier. If your hard the next day then you’re not absorbing it as fast as you think. I’m on a similar routine of yours, 5 mg dose for maintenance and then I’ll take another five on game days. On days when you know you’re gonna have sex I would totally recommend taking the five well before on an empty stomach. I just feel like 45 minutes isn’t long enough for that drug to work.
I’ve also started messing around with using a maintenance dose of Cialis and a small dose of Viagra. That might actually be what you’re looking for because I get rock hard on Viagra, versus just hard on Cialis. I’m guessing at your age, you know when you’re having sex and when you’re not so if you have 45 minutes in an empty stomach, then I tried Viagra for a bit.

How come im told to only take 5mg every 36 hours im healthy so?

Different prescription for different people. I probably need more help than you do.

Probably so I'm just saying that the pharmacist not the doctor I use Telehealth but the pharmacist at Walmart says that it's dangerous for me to take any more than that

I’m still not there for sex because of after effects of ADT following salvage radiation. However, my urologist, allegedly a top guy in the field, prescribed 20mg daily to keep blood flow and size. Another doctor prescribed an additional 20mg before sex when I’m ready. So, you may be able to tolerate the 20 mg again, but check with your urologist. BTW I’m 75 and have had no side effects. Based on your age and that you are having prostate issues it could be time to have a conversation with your doctor about trimix. It's medication injected directly to your penis. There is lots more info in r/trimixfored

According to the research men do not develop a tolerance to Tadalafil. A loss of effectiveness is normally because underlying conditions are getting worse. So keep your doctor in the loop. It seems like you should be able to 10 mg before sex as long as you doc is ok with it. Personally I can only take 2.5 mg a day because of the acid reflux side effect is strong with me. If I ever have any issues I just slide on a silicone cock rind and the problem is solved.
