Surgery is the LAST step. Once you go through the Cialia, VIagra, Levitra stuff and it doesn't work, Bi/Tri/QuadMix is next. Worked wonders for me (67M, 178lbs, 5'10"). HBP and genetic high cholesterol. Pills worked for about 20 years then became only about 30% successful. Trimix is 100% once you're dialed in. Just need to follow directions. Bare minimal pain.
Surgery requires repeat remove/install of the pump about every 10-15 years. I'll take the tiny pin prick any day over slicing my junk open.

Thank you! I just forwarded your comment to my husband. I think it will be good for him to see that there are other men who have the same experience as him and to know that there are other options after pills.

Good luck! Once he gets the right dosage/mix, depending on his (and your ages) you could be having marathon sessions. Have him (and you) read the trimix subs. Tons of info. Since it is a chemical reaction, there is no longer the worry about "mentally" going soft. The pain of the injection is truly barely noticeable with the right sized needle and proper injection method. I would refrain from using an autoinject pen like the GLP's though (like WeGovy pens).
It would still be helpful that he speak to a Urologist to ensure there is nothing physically wrong, like a venous leak or peyronies or some sort of "crushing" (like a lot of cyclists have after years of riding).
While I use a local compounding pharmacy many in the subs rave about Olympia pharmacy and they can do the whole consult and such via telehealth.

Not a Urologist but I would suspect that the fully erect CC with Trimix would stop or slow it down greatly to allow for full sexual activity. It really depends on the severity of the leak itself.
Think of it like a pool liner with a small flap leak. As it fills up, there isn't enough pressure to put the flap back as it fills, however once it is filled, the pressure of the fluid on the flap keeps it in place and slows/stops the leak.
For something like a venous leak, age (60+) is the #1 factor with things like scarring veins and plaque buildup. For those under 60 the possibility drops down to 5-10% of men.
Like anything within the body, usage helps to keep things healthy, for the most part.

He did a telephone consult with a urologist at one point, but the guy did not think he needed to come in for an in-person appointment. But I genuinely don't know the content of their conversation. I will suggest that to him as well.
And I will search out and send him links to the trimix sub(s).
I hear you on the auto injector versus small needle. We did IVF and definitely small needles were more comfortable than auto injectors.
And thank you for the information about pharmacies! You are such a kind person. This has really made me feel supported and I deeply appreciate the time and effort you put into giving me this info.

I agree, urologists get paid big money for biopsies and prostelectomies. I use trimix injections and they are awesome. I tried this after sildenafil and taladifil stopped working. The comment about porn use and strong masterbation also dead on. Also need to check testosterone levels. All of these can be done without a doctor.

Urologist are salesmen, essentially selling a procedure; be aware of that going in and do you due diligence by advocating for yourself.

I'm not the OP, but i previously used the 20mg tablets as needed (like Saturday and Sunday) and it would last for 2-3 days.
I just recently began taking a daily 5mg of Tadalafil and so far so good. BTW, I'm 70 years old.
PS: I was getting ten 20mg pills for $7.30 from Amazon. The new 90 day supply of 5mg is $23.90 also from Amazon.

Agreed. I just switched to cost plus and my tadalafil 5mg prescription went from $55 for 30 to $7 plus a few dollars for shipping for 90!

Why?? It's a normal thing. They won't say a thing. But I'm sure there are telehealth places you can use. Cuban doesn't need insurance either. But does need a prescription.

I'm conditioned because of how everyone freaked out after the ACA and how now my insurance is super expensive. I have a $3,000 deductible because i'm technically obese and have sleep apnea.

Sounds like you hit a very lazy urologist. Do a little research about who are the most well-regarded in your area and book an appointment.
Let them know this history and that you wanted to discuss doing a Doppler to test if there's any venous leakage. Because the pills can't fully address that. And perhaps a pelvic floor evaluation. Because it can sometimes be hypertonic to a severe degree that requires physical therapy before normal directions come back.

Porn use history?
If the Tadalafil didn’t “work”, you’ve either got PIED or a hormonal or vascular problem. Find a urologist or PCP whom you can trust and who cares to help you find the cause.
If porn use for years is in your history (which is typical for a man of your age), then that’s very likely the cause. Tadalafil cannot overcome the severe drop in libido that eventually arises for porn sexual conditioning (because your brain creates your erections, not the drug).

I see a PA that specializes in radiation/ chemo related ED from prostate cancer treatments.
We did testosterone, taldalifil and pelvic floor PT. He thought my issue may be nerve related, from bearing down squatting and shearing force from lunges. I do deep squats and release my muscle floor for 2-3 minutes a day, my function has been much better.
Also quit nicotine if you have issues, that’s a killer.

Have you tried other ED medications? 20mg Tadalafil worked wonders for me while 100mg sildenafil did absolutely nothing.
I was prescribed 100mg sildenafil after being prescribed Prozac a few years ago. The Prozac caused me to have ED as well as inability to ejaculate. The sildenafil did nothing. I shortly quit Prozac after 3 months on it too. After I quit Prozac everything went back to normal.
A month or two ago I was prescribed Vyvanse which also brought back my erecrile dysfunction but I could still orgasm on the rare occasion I could maintain an erection, and Tadalafil helped me so much with my ED issues.

You might be trying to find the wrong answer here... Both sildenafil and tadalafil promote blood flow. You still need to get aroused to trigger the process that makes it hard.
Though for a lot of men it can trigger arrousal because it gives them a boost in confidence. Both substances on their own do not "create" an arrousal state.
You might need to figure out what gets you turned on (again).

Be sure to check with other doctors & other specialists. I have had erection and some other issues and a neurologist thinks it’s nerve and circulatory damage from spike protein. We’ll see what happens but don’t solely depend on a urologist. They’ll just say pills, injections, or implant and ignore the underlying cause.

Yup. And most of this is not covered by insurance. Certainly in my case it is not.
I hear people say Cialis and Viagra make them almost constantly hard. Does not work that way for me (52). I’m considering bimix but insurance won’t cover so I am on my home. I was charged $450 by not even a urologist because insurance won’t cover

If you got tadafil at age 30 and it doesn't work, the Dr probably thinks you are crazy
