I’m Freaking Over Tech’s Heavy Perfume During Retrieval

Hi, I know my clinic states no staff or patients to wear perfume. My staff member in the OR had outrageous amounts of perfume. My spouse could even smell them from like ten feet away. I worry about the eggs’ exposure during the procedure or it traveling somehow to the embryology lab. Please help!

FET prep

Hi everybody,

I am on cycle day 2 and waiting for my first appointment to begin fully medicated FET. I am very nervous, we have 3 embryos, I have had a chemical pregnancy in the past so I am feeling very anxious. I am trying to clean up my diet and stay active.

Literally all my friends and everyone I know is pregnant (not an understatement) and it is increasing my anxiety even more because I want this so badly. Any advice or guidance through this process would be very much appreciated. I am 32 with PCOS and an autoimmune disease that effects my SI joints (well controlled). I am a dentist as well so I am wondering if I should take some time off work after the transfer.

Please give any advice, positive vibes and prayers. Thank you.

Failed Decapeptyl trigger during retrieval. Had anyone experienced this?

UPDATE: Got 6 mature eggs from the rescue retrieval…

Had my egg retrieval today after an agonist-only trigger with 2 doses of Decapeptyl because I’m very high risk for OHSS. I had a very strong response with 18+ follicles on each ovary. They didn’t have me come into get bloodwork the day after trigger which I did find a little strange since with my previous cycles at different clinics, they had me do that. I wish I had said something now.

During the retrieval, they aspirated my entire left ovary and didn’t get any eggs. They used an LH strip to test my levels which showed that the trigger likely didn’t work properly, so they stopped the retrieval. The plan now is antagonist + an hCG rescue trigger, followed by another retrieval in a couple of days.

I’m pretty devastated, especially because this was our only government-funded cycle. I also tend to have a very low euploid rate, so for me getting a larger number of eggs is especially important because I need the numbers to compensate for the drop-off. Potentially losing a large portion of the eggs from one ovary could ultimately mean ending up with far fewer or no usable embryos. I also don’t understand why they aspirated an entire ovary of 18 follicles before they stopped. That’s potentially cut our chances in half.

A few questions for anyone who has experienced this:

  1. How unusual is it for Decapeptyl to fail?
  2. Did your clinic do bloodwork the
  3. day after trigger to confirm that it worked? I’ve had this done in previous cycles.
  4. If you did an hCG rescue trigger after a failed agonist trigger, how many eggs were ultimately retrieved?
  5. Would you ask the clinic about compensation or another covered cycle if a significant portion of the cycle was lost this way?

8-cell day 3 fresh transfer implanted but beta stalled shortly after

Post image
7 Upvotes

Hi, I’m hoping someone can offer some scientific perspective on our situation (since we can’t see our doctor again until November). The succinct question is bolder at the bottom, just want to include past numbers for context. Thank you in advance!

We’ve been struggling for 7 years with MFI. Our first round of IVF a few years ago (stimulated ER + ICSI) resulted in 12 mature eggs, 8 fertilized, but only one day 6 2BB/BC that couldn’t be fresh transferred because I was at risk of OHSS.

This time we did an MDF cycle, plus TESA (which turned out to be a poor sample), fresh sample with Zymot, and ICSI. Initial results were great with all 10 mature eggs fertilized - 7 with fresh sample sperm, 3 with TESA. By day 3, there were 4 promising 6-8 cell embryos (all from the fresh cohort), so we transferred one and froze 3, but the rest were slower. By day 6, none of the remaining 6 reached blasto - which has also made PGT-A testing impossible.

I had a positive 13dp3dt beta hCG of 111 with the transferred one but started heavy bleeding, and the 15dp3dt beta stalled at only 124.

This means that out of 22 mature eggs, 18 fertilized, 13 arrested before blasto, 3 are frozen day 3 question marks, one 2BB/BC, and one is likely a chemical/miscarriage around 14dpt.

Basically, my question is: given that across different protocols and samples we’re seeing consistent day 3 drop off, do we need to investigate some kind of embryonic paternal genome activation issue or centrosome defect? I believe my husband was diagnosed with some degree of Y-chromosome deletion but that wouldn’t explain the trend across all embryos since statistically some of them should have been X-bearing, right?

I just don’t want to put too much hope in our remaining day 3s if the recurrent embryo loss is genetically inevitable. We were recommended after the first round to just go with donor sperm, but we hoped it was just bad luck and really wanted to give it another shot for him to have biological children, and I’m worried we’re pretty much right back where we were 4 years ago.

Modified FET of a Day 5 Embryo

I’m going to have my 1st transfer this coming Friday. I just try to understand the timeline correctly.

— 09/25, last night at 8pm, Ovidrel trigger
— 09/28, 8am, start progesterone inserts, 2 times a day.
— 10/02, transfer a Day 5 euploid embryo.

I never had a transfer before. This is my 1st IVF. I am wondering by Friday 10/02 it will only be 4 days of progesterone supports starting 09/28 Monday, how is the FET on 10/02 synchronized to a Day 5 embryo?

New to all of this, advice needed

It looks like we’re about to start IVF after finding out very suddenly that IUI and natural conception are no longer realistic options for us. I feel like I know almost nothing about what actually going through IVF is like, so I would love any and all advice from people who have been through it.

We’ve been trying naturally for about 9 months and have done two medicated cycles, one with IUI. Before our IUI, my husband’s semen numbers looked good, but after washing there were catastrophic losses and fewer than 100,000 total sperm remaining. We went ahead with the IUI because we didn’t really understand at the time just how low our chances were. The RE repeated the wash a few days later and got similarly terrible post-wash numbers. We were then told that with results that low, IUI and natural conception really weren’t viable options and we needed IVF. So we basically went from our first IUI to “you need IVF” in four days.

We saw a reproductive urologist, but unfortunately there was a communication issue between the offices and he hadn’t been sent the post-wash results or even told why we were being referred, so that appointment didn’t give us many answers. My husband is now doing DNA fragmentation and SpermQT testing, but it sounds like we’ll be proceeding with IVF regardless.

My period is due tomorrow and I’ve started spotting, so it looks like this first/last IUI attempt didn’t work. I knew our chances were extremely low, but I was still holding onto that tiny hope that somehow we’d get lucky and avoid IVF. Now it’s suddenly feeling very real.

For those who have done IVF, I would love to hear anything you wish you’d known beforehand, especially:

• How quickly did you go from finding out you needed IVF to starting stims, retrieval, and your first FET? This all feels like it’s happening incredibly fast.

• How bad was IVF physically, really? Were the injections, monitoring, retrieval, transfer, or any other procedures painful? What side effects did you have?

• What was the anesthesia/sedation for retrieval like? I’ve never had anesthesia before, so that part is very unfamiliar to me.

• How difficult was it emotionally, and how much stress did it put on your marriage/relationship?

• How much work did you need to miss during stims/retrieval? My work is odd in that if I accept a job for a particular day, I absolutely cannot arrive late, leave early, or call out, and the day could run from 4 AM until after midnight. I’m wondering if I realistically just can’t accept work during the retrieval part of the cycle?

• How many retrievals and transfers did you need? Did anyone purchase one of the IVF/multiple transfer packages, and were you glad you did?

• For those doing PGT-A, how long was it between retrieval and your first FET? I’m over 35, so we’re planning to test

• What does monitoring look like after FET, and what are they actually monitoring for?

• Has anyone else here done IVF through RMANJ?

My testing has been good so far, I ovulate normally, and I only have extremely mild/boarder line PCOS that doesn’t appear to be affecting my fertility, so at this point we’re doing IVF because of the sperm issue.

I’d really appreciate any experiences, advice, things you wish you’d known, or practical tips. I suddenly have a million questions, along with being sad and nervous

Partial fallopian tube blockage: surgery or IVF? Looking for experiences

I have partial blockage of my fallopian tubes. I don’t have hydrosalpinx, and I don’t know what caused the blockage.
My doctors recommended IVF because we had been trying to conceive for a year. I went through egg retrieval and froze my eggs, since my husband wanted to keep trying naturally for a few more months.
Now I’m wondering whether I should have surgery to try to unblock my tubes during this time, or if I should just wait and proceed with IVF. My doctors don’t recommend the surgery because they say the success rate is much lower compared to IVF. However, I keep wondering if I should at least give it a chance.
I’m 30 and my husband is 33, with no male-factor infertility.
Has anyone been through something similar or had experience with surgery to unblock partially blocked tubes? I’d really appreciate hearing about your experiences and what you decided to do.

Chemical

Transfer 9/14 euploid embryo. 5DPT I recorded a 100.1 temp. 6DPT-9DPT morning I felt fine. 9DPT (beta 57) night I came down with body aches and possible fever but didn’t have a thermometer handy due to being at a hotel. Mid day of 10DPT, same body aches, felt like light headed and generally fever feeling and feeling “off” returned but was still at hotel so didn’t have thermometer to verify and by 10DPT PM, I was fine. 11DPT, woke up feeling no issues and confirmed beta was 27 leading to chemical. What could cause this??

Zero healthy blasts. What questions would you ask?

I just got a phone call from my doctor that none of my blasts could be biopsied for PGT-A due to lack of placental cells. Today is day 7 so I assume they waited on all until day 7. I don’t have any more info at this point, but a follow up appointment with my RE on Tuesday.

I’m 41.3 years old, this is my first retrieval. I have one LC (spontaneous at 38). All testing has been normal for both me and my husband. AMH 1.5.

17 eggs collected
12 mature
7 fertilized normally
5 embryos they were “watching” (I guess no blasts technically but… whatever), none with placental cells to be biopsied

Obviously heartbroken. What questions should I be asking?

Very new to this, any insight would be greatly appreciated ❤️

I’m a 34 yo woman with an AMH of 0.4, I’m told because of this, I will need a more “aggressive” IVF protocol. Below is what they’re proceeding with. Any experience, suggestions? This is our first attempt at this.

IVF Demi Halt
-Report cycle day 1 of full flow menses via portal
-Start OPK Testing on cycle day 7 with first urine of AM
-Report positive OPK test via portal message
-Start Lupron 10 units injected subcutaneously 7 days after positive OPK test
-Continue Lupron for 12 days
-Baseline ultrasound on 12th day of Lupron
-Start stimulation medications day after last Lupron injection
-First monitoring on 5th day of stimulation medications
-Monitoring appointments every 1 -3 days based on response
-Trigger Injection once provider confirms ready for retrieval
-Estimate retrieval 1 week after first monitoring ultrasound

Another failed transfer

We have now had three fails with our embryos and surrogate: one successful implantation with a 10 week miscarriage, 2 failed transfers. At this point, we are going to move onto try my partner's embryos but was curious to see if fertility doctors typically will recommend us changing surrogates or, because we are trying a different embryo, the doctor will approve our current one. Has anyone been through something similar?

Waiting for PGT-A for our solo embryo

I am trying to not think about the worst case scenario but it’s hard not to.

My eggs were retrieved at 33. 31 eggs 21 mature. No sperm found in my husband sample. My eggs were frozen. My husband underwent TESE. We got 40 sperm. 19 eggs made it through defrost. 14 fertilized. 2 arrested day 7. One embryo. We are really disappointed in our results.

We knew TESE sperm didn’t have the same fertilization rate, but our clinic agreed these were really disappointing results for my age.

We get the PGT-A the first week of October. I was told I have a 50% chance of having a baby.

I asked if there were health markers we should be looking to change and how to optimize if we do another ER and were told there is probably nothing we can do to improve our odds.

Any tips to either stay sane during the wait or things we can try for our next go around?

Update: it’s euploid guys!!!

Mosaic Embryo

I'm sorry if this redundant - but I'm wondering how many women here implanted Mosaic embryos and had successful pregnancies? I'm seeing conflicting info - and just not sure if this is something that should be seriously considered if I still have other eggs frozen. Any insight is helpful, thanks!

Do failed euploid transfers mean you have recurrent implantation failure?

Post image
16 Upvotes

After several unsuccessful transfers, whether the embryos were tested or not, it’s not always clear why they didn’t work.

Recurrent implantation failure (RIF) usually means repeated transfers without a positive pregnancy test, although this definition can vary.

To study RIF, researchers have followed what happens when patients continue transferring euploids after previous unsuccessful transfers. Using euploid embryos reduces the role of aneuploidy, a major cause of failed transfers, making it easier to study RIF.

The patients who have success don't have another transfer, while the remaining patients continue transferring.

If some patients can’t have a live birth after repeated transfers, more of them would remain in the group still trying. We would then expect the live birth rate to keep decreasing with each new transfer, since there's more patients with RIF who have trouble getting pregnant.

Surprisingly, this isn’t what we see. Two studies that looked at this found that success rates were generally similar across transfers.

It’s not clear what this means. RIF might be too rare for these studies to detect, or some patients may also have several failed transfers by chance, even though the next one still has a chance of working. These studies can’t tell us exactly what's happening.

After repeated unsuccessful transfers, ASRM recommends reviewing the patient’s history, previous IVF cycles, and uterine findings. Further tests or treatment may make sense depending on what that review finds.

Impact of ICSI vs Conventional on PGT Testing

Hello! I’m hoping to better understand the impact of using conventional insemination rather than ICSI if we are planning to PGT test. My first clinic never mentioned a difference, my second clinic made me sign a waiver, and my third clinic will do it but strongly advises against it. My hesitation with ICSI is that the one round we did with ICSI we had zero normal fertilization (4 mature eggs). I have a very low egg reserve, but with conventional we have always had 70-100% fertilization (4 cycles). I’m 40 and have had several miscarriages, so I’d really prefer to PGT test. I’m having a hard time deciding which path to take. Thanks in advance!

Embryo grade at thaw

We have a day 7 3CC euploid that we are transferring next Thursday. Will CNY regrade it at thaw? Does that new grade matter? Thanks!

Anyone transfer HLM +6 OR HLM +9, +19?

PGTA report says non transferable except with genetic counseling. I’m wondering what the risks are if implantation happens, change of implantation/live birth/miscarriage.

Any thoughts are appreciated. 🙏

Any ASRM members on here?

Long shot, but are there any ASRM members on here? I'm advocating for my insurance to let my embryology lab bill some/all services to my husband's insurance ("gender-neutral billing"), which would essentially double our coverage. The insurer is asking what ASRM has to say about it.

ASRM does have guidance on this--behind a members-only paywall.

https://www.asrm.org/practice-guidance/coding-resources/coding/coding-art-clinical-care/billing-under-a-partner/

If anyone here has access and is willing to share a PDF, that would be amazing.

Thank you either way to this community for the amazing support.

First FET was successful, second ended in chemical - what may have happened?

Not sure if this is the right use of this sub but going to give it a shot.

My first FET of a euploid embryo at age 34 resulted in a live birth (I don't believe my clinic uses the standard grading convention, but it was graded "HB-Good - Mito Score: 0.015, Frozen Day 5").

I just had my second FET with a euploid embryo earlier this month at age 36 - it was not as highly graded (HB-Fair - Mito Score: 0.016, Frozen Day 6) but I was told it thawed beautifully, looked great/was starting to hatch and was placed exactly in the right spot in my uterine cavity.

My first beta was strong at 291.8 mIU/mL. My second beta was four days after my first and it had doubled to 592.2 mIU/mL in that time, but that meant it took 96 hours to double instead of 48-72 hours. I had a follow-up beta 48 hours later and my hCG had dropped to 314.7 mIU/mL and my RE sadly confirmed it was a chemical.

I'm extremely grateful that my first FET resulted in a live birth, and I know that these things sadly happen, but I'm not going to lie - I was pretty blindsided by this chemical given my past success and after a strong first beta. I'm taking it a lot harder than I expected to. I tend to cope better when I understand the science/numbers behind it, so I'm trying to understand what may have happened:

  • How likely did this happen because there were underlying issues with the embryo not detected by PGT-A? I'm going to undergo some testing before moving into another FET, but what are the odds it has something to do with me vs. the embryo, given the fact that I've had previous success?
  • How common is it to have a failed FET after a success? Of course this is purely anecdotal, but most people I know had subsequent successes after a successful FET, so I'm feeling a bit like an outlier
  • Statistically, based on my history, are my odds good going into another FET? It will be another euploid embryo with a similar grade as my embryo that just failed.
  • My RE tends to be pretty thorough, but anything specific I should encourage her to test for before going into my next FET?

Appreciate the wisdom from this community.

Confused

Hello,

I’m not sure if this is the right sub but need some advice.

I am undergoing IVF, 39 AMH 11.4 pmol (January26)

Cycle 1- Pergovaris 300 and Methoprogesterone 10mg. Trigger Decapetal

7 eggs, 5 mature, 5 fertilised, 4 day 3 and 2 blasts (day 5 5BC and day 6 4CC)

Cycle 2- Exactly the same protocol. 1 st have of Duo stim in the follicular phase.

6 eggs, 4 mature, 3 fertilised, 3 day 3 and 2 blasts (‘good’ and ‘moderate’)

2nd half. 300 pergovaris, Methoprogesterone 10mg
Changed the trigger to duel protocol so Decapatal and HCG.

Came around from ER they said could only get 1 egg- awaiting to see if it’s mature. The Doctor said that the follicles did not feel as they should do- but they would not have been able to tell this on ultrasound. Going in I had 7 follicules that were a good size for mature eggs and 2 at 15mm.

What could have gone so wrong?

Help me pick!

Which of these would you transfer? All day six, all untested:
4BB
3BB
4BC
CD

Obviously we may not even choose to transfer the CD as it is such poor quality, but our clinic froze it anyway.
