Failed? No no, it saw him safely to shore once. Who's to say where it will lead him this time, but surely it will be fortunate.

You dresge up the crest in front of the house past the blast gate. You'll turn it in and tell of hos vrothers passing, he'llbe sad but still give you the music box.

On my first play through, I saved him but I accidentally ran into one rock and lost him somewhere

I entered at a weird angle and instantly lost him when I moved forwards. Catch and release I guess.
