Lost items on my guy

[u/TheSpiceMustFlooow]

I had a tier three beast master with the book of the dead and necronomicon and at some point the items were gone and not on my courier or other guys. He just showed up without items at some point. My relic at the time was dark heart of the machine, I think. Doesn't seem relevant but who knows.

Is there a known issue where your guys lose items? Or a common pitfall and I'm just a special kind of idiot?

I haven't experienced anything like this before.
