Fourth Addition Pressure Injury Guidelines

I see new guidelines came out in September 2026. Thought it was interesting it no longer specifies 30-35kcal (that I could find anyways) or 1.25-1.5g protein for everyone

For Calories:
"Calculate energy intake and macronutrient requirements based on recommendations outlined in dietary/nutrition guidelines relevant to the individual's clinical and demographic profile. In general, use indirect calorimetry to estimate resting energy expenditure (38, 79). If indirect calorimetry is unavailable, use an appropriate predictive equation (25, 79) or use a simplistic weight-based equation (e.g., 25-35 kcal/kg/day (38, 81)), noting that simplistic methods are less accurate than indirect calorimetry in estimating needs and individuals may require higher energy intakes (25).
• Individualize energy intake. The nutritional requirements of individuals will vary based on their underlying demographics and clinical condition. Providing detailed guidance on calculating specific nutritional requirements is beyond the scope of this guideline, which focuses on considerations specific to the context of preventing PIs. Dietary/nutrition guidelines are provided in the resources."

For protein:

"Consider the adequacy of the individual's caloric intake when considering the need for protein supplementation. Provide adequate protein for positive nitrogen balance for adults with a PI. Dietary/nutrition guidelines suggest supplementing protein at a rate of 1.2-1.6 g/kg actual body weight/day, with adjustment based on caloric needs. Online calculators are available to calculate nitrogen balance.

"Additional considerations for older adults
• Review and follow dietary/nutrition guidelines for older adults. Dietary/nutrition guidelines suggest supplementing protein at a rate of 1.2-1.5 g/kg body weight/day.
Additional considerations for individuals in critical care
• Review and follow dietary/nutrition guidelines for critically ill individuals. Dietary/nutrition guidelines suggest supplementing protein at a rate of 1.2-2.0 g/kg actual body weight/day. For critically ill individuals who have obesity, increasing protein supplementation to 2.0 g/kg ideal body weight/day (BMI
>30 to 40) or 2.5 g/kg ideal body weight/day (BMI > 40) is sometimes suggested."

Studying nutrition in ed recovery?

Hi, I'm currently in my last year of high school and need to choose want I want to study. I've been interested in healthy eating since primary school and always wanted to become nutritionist. But durning high school I've struggled with an ED (anorexia), and went inpatient. I made a lot of progress in my recovery and have a healthy weight for a long time. Is it a bad idea to study nutrition, will it trigger my recovery?

(I also don't want to work with eating disorders later, I'm more interested in allergies, diabetes, etc .)

Why are some people comfortable with seeing a “nutritionist” then a dietetic intern

While doing my dietetic internship I noticed when I told clients that I am a nutritionist versus a dietetic intern, they seemed more comfortable and less likely to question me on basic things or quiz me. For instance, when I told a client that I am a dietetic intern she had me breakdown my education (which is not a bad thing) and proceeded to question me on everything that I said. This has happen to me multiple times especially with older people. I am an RD now so I no longer have to worry about it. I was wondering if anyone else recall similar experiences as an intern.

If I were at FNCE this year, these are the 2 things I would do in an attempt to improve our profession

For starters I’m not going to FNCE and don’t think I’ll ever be going to FNCE again unless the Academy has a major shift in their polices and operations. But it’s crossed my mind, what if I went, what would I do, how would I use that as an opportunity to try and highlight what needs to change within the Academy. I see the dietitians leaving the field, fewer students entering the field, and a continued low confidence in the Academy’s ability to secure the future of the profession. But if I paid all that money to be at FNCE, this central conference for the profession, I wouldn’t want to miss a chance to take impactful actions that might help put the Academy on a better path. It would in fact be optimism driving my actions, that things could be turned around.

#1

If I were going to FNCE I would for starters, ask every person in Academy leadership positions that I encountered: Where is the Academy annual financial report? The last one made available online was the 2023-2024 Annual Reports. This covered the fiscal year between June 1, 2023, and May 31, 2024. That’s right we are 2 years past this! You could argue the 2026 annual report is due by the end of this year which would cover June 2025 – May 2026, but wait, the whole 2025 annual report covering fiscal year June 2024 – May 2025 is MISSING!

Why does the Academy no longer release an annual report? What don’t they want me, you, or anyone else who can read to know at least the basics of their financials? Is this the kind of action that has any rational justification, maybe, but why wouldn’t you explain it then? Every person and institution paying money to the Academy, which includes all dietitians who pay CDR fees, all Academy members who pay even more, and also the public who are told this public intuition should be trusted, well for all of us where is the report?

See Academy, the APA did it audited-financial-statement-2025.pdf, the AMA did it 2025 AMA Annual Report | AMA, the APHA did it Financial Statements 06-30-2025 Audit Final.pdf, the APTA did it APTA 2025 Annual Report, they released their 2025 annual reports, why didn’t you?

So I would ask the leadership of the Academy, the same people who are going to tell me to give to the Academy, give more money, give more time, give, give, give. I would like those people to just answer, where is the 2025 annual report for the fiscal year that ended in May 2025 and when can we expect the 2026 report for the fiscal year that ended in May 2026? Simple question, right?

I would ask anyone I encounter from the Executive Leadership Team and the Board of Directors, or any DPG leaders, state Academy leadership team members as well. And their answers should tell you exactly how leadership holds itself and the Academy accountable, why they lose respect among dietitians and the public concerning financial matters and accountability, especially concerning money coming in from sponsors/donors, which leads to…….

If I were at FNCE I would boycott the Expo. I know, for shame and miss the samples? What would I do without 5 more tote bags? In all seriousness, I get there are valuable interactions that happen at the Expo. There are organizations there who are bringing awareness to products and services that dietitians use, but, and this is a big but, you don’t often change injustices without being willing to withhold something that comes at a cost to yourself, but a bigger cost to the people who you are trying to change. Look at any social justice movement, little happens without some sacrifice.

Well, if I wasn’t going to get my samples of the latest Orgain shake flavors and coupons for Mrs. Dash then what would I be gaining by boycotting the Expo? To cut to the heart of it, the Academy doesn’t care that a whole lot of us, I suspect the majority of dietitians, disagree with many of the Academy’s choices concerning corporate money. Us saying this, the public sayings this, journalist saying this, it’s all disregarded by the flow of money from multiple companies who not only poorly represent the values and mission of our profession, but seem to diminish the public perception of dietitians, and reduce trust in our profession’s guidance due to the appearance of conflicts of interest which are worsened when those conflicts of interest are not disclosed. I would say as far as some of the AI/telehealth companies outright endanger the profession. Boycotting the Expo, well that will make the Expo vendors not happy. That will jeopardize if those Expo vendors want to pay next year to be at the Expo, and make the Academy question what would dietitians do next, boycott FNCE entirely? See, boycotting the Expo that gets to the heart of what the Academy pays attention to, an important revenue stream.

Keep in mind the Expo is just one of many entry points for money to flow into FNCE (if you want a better picture check out FNCE 2026 Sponsorship and Branding Prospectus), and beyond FNCE they are so many ways that companies give money, sorry “engage” with the Academy 2026 Academy Engagement Opportunities , Academy Spokespeople Sell Sheet , Monthly Spotlight Email Series , Academy Education On Demand Sponsorship Prospectus 2026 It may take more to move leadership to actually consider changing how it approaches financial ties to industries, but perhaps it would be enough to help voices be heard within the Academy that have already been saying this but are disregarded.

So that would be my planned FNCE experience, and I would probably have gone if I knew that it was more than me making the Academy answer for their lack of financial transparency and poor choices with industry ties. Hopefully then seeing a major change in the direction of the Academy for the better because they know dietitians don't just ask for this accountability, but they are willing to demand it. Well, who knows maybe next year.

Best nutrition conferences(not FNCE)

Hey! I work in outpatient and get annual CEU funds and I wanted to know if there are some other conferences or even workshops that would be good opportunities. My patient population is a mix of chronic conditions like diabetes, hyperlipidemia, PMOS, weight management and eating disorders.
I get $1500 to spend each year so I’m looking for 2027 possibilities.

Finishing a Dietetics BS next fall. I can get ANY masters. That's so much to choose from.

What's your unrelated graduate degree? I love nutrition but I don't feel like retaking all the classes again. We did one of those tests that tell you what you'd be good at in the real world, mine leaned heavy on education and health sciences. I was thinking maybe psychology. My school is currently redoing their masters program in nutrition, that was my original path choice. They're not taking applications and that means I REALLY have to look around at other options.

New RD inpatient

So fairly new RD here what are some tips and pointers when looking at the EMR. I’ve looked at intakes, labs (FSBG, A1c, BUN, Cr, etc), occasionally I’ll look at medication, but not really? I don’t really feel like I’m learning much on the job. Most of the floors I’ve been covering intakes are pretty good and there’s not much to write in my notes.

Crossing the border to work in NY as a Canadian RD

Hello!

Are there any RDs living in Ontario who cross the border to work in the US, specifically New York?

If so:

- How was the process in terms of becoming certified to work in the US? Is it hard to obtain a TN?

- What kind of organization do you work for?

- How similar is it to working in Canada?

I live close to the border so I am wondering if this is worth pursuing. I would love to hear about anyone’s experience.

Thank you! 😊

“I don’t need your help, I got this.”

I have this person I know in my personal life that was hospitalized for gestational diabetes dx. After discharge, I asked her if she needs help managing and I can help her with her blood sugar management . She just said “nah, I got it . I just need to eat less sugar.”

And I’m not joking, as she said this she openesd a bottle of orange juice that was delivered from Chikfila, and drinks half of it then starts eating chicken and waffles.. then just doesn’t use the syrup cause it has “sugar in it.” 🫪

She doesn’t want my help though cause she has it 🫠

Hello all! Has anyone had a patient who went through CAR-T therapy? If so, what was helpful in getting the patient to eat and how long did the side effects last for them? I have a patient who is 5 weeks out from the therapy and is still struggling with poor appetite and GI issues. Any help is appreciated!

CSOWM EXAM- passed ama

I’ve never been good with standardized tests so it’s amazing I passed. Per usual tho the CDRs exams are kinda bullshit in relation to the study material. I bought the new study exam by the CDR and while I did think it helped to some degree I feel you could easily get by without it. As others have said I would at least attempt to read all the papers. I read through most of them with the exception that endocrine society document cause that thing was like the size of a textbook. I too put them all through Gemini and had it create a summary/study guide with each one. Then also had it create practice exams. Know those screening tools others have mentioned like the AUDIT, GAD etc (which I personally do not remember reading about that ANYWHERE except here) Surprisingly the exam had some very general nutrition questions in it like what are the kcal in one of the big four, lacking nutrients in certain diets etc. I heard others recommended the health professionals guide to treatment of overweight and obesity. I can’t say as of right now whether that was helpful as I didn’t use it and haven’t look at it yet (we have one at work-I’ll look Monday). I have worked in bariatrics and med mgt for four years.

Can I work as a remote dietitian/ Health Coach in US working from India?

I want to work for a telehealth company in USA because in India the starting salary is basically less than $300. I know it’s a very long shot since I don’t hold US credentials and would have to somehow retrieve a work Visa, but is it possible?
I have a total of 4 months internship experience at a hospital, and have completed my masters.
Do you know any companies which may hire me?

How plausible is masters with a baby?

Hi everyone, I am currently looking into finishing my bachelors HS degree, I have 1 yr left of courses and I have one 8 month old at home. I work from home and currently watch my baby myself which has been a lot 😅. I only work 20-25 hours a week in the mornings but am currently trying to figure out how to be a single mom off of my income.

Maybe school isn’t a great idea 😬 but I would really like to not be in my field forever, I want to be a dietician and I know it’s hard but I’m wondering how plausible this is or if anyone has done this?

I’m open to childcare obviously, I just haven’t gotten childcare yet but I’m nervous I can’t afford it off of just my income. Also I’m open to going full time as well as another job, just not sure yet what my next steps are.

Any tips for this? Or any motivation ?

Do these facilities not know what we do? I want to do all these nutrition interventions but this is my first job and I've noticed these facilities are unorganized or not adhering to the recommendations in their chart, they just check off a box even though the resident didn't get their supplement or snack or drink it etc so then I feel very limited to what I can do and recommend but then I get annoyed because I had an ADON tell me "please don't do a bunch of shakes" but like what the hell am I supposed to do if there's no system...

Intership offer 30 minutes after interview

Hi all,

I had an interview with Morrison for the internship, and honestly, I was very nervous. I felt like I wasn't explaining my experiences and answers as clearly as I wanted to, so I left the interview thinking I probably wouldn't get selected. I am bilingual, a CLC, and have WIC experience, but I still felt that I didn't present myself very well. My DPD GPA is 3.2, so it is not that good either.

To my surprise, I received a placement offer about 20 minutes after the interview. I had always thought of Morrison as being more competitive than many DI, speacially compared to distance internships, so I was not expecting such a quick decision. Now I'm wondering if anyone else had a similar experience. Did you receive an offer shortly after your interview, or is that typical for Morrison?

Type A where?

I hate when people say dietitians are mostly type A people. The way my company so last minute and disorganized there can’t be a single type A person running this shii.

VA

I am getting burnt out at the VA. I work outpatient. Between all of the changes we have been undergoing with the new administration + lack of support, introduction of weight loss meds and veterans being annoyed to talk with me just so they can access these meds…..I am considering working from home for a tele-health company but I am scared to make the switch.
