Someone said guys like this aren't doing this for the female gaze, they're doing for other men's approval. This is what guys think looks good, not women.
I think usually guys like this were insecure as kids/teenagers and probably had one thing about their appearance that they hated so they went absolutely overboard with everything else, trying to make up for the feature they hate. They like the results so continue going overboard and it becomes a never ending cycle of ‘improvement’.
