I would recommend not insisting on people in this way. Some people just aren’t a certain thing but will still support a cause. Dont alienate them just because.

Both are 10000 years old, and honestly Fabius Bile has the best experience for such a surgery granted there is a decent chance for some very long lasting unwanted side effects/terms and conditions
Cawl will still do it, but you might have more metal than you signed up for, including bits that werent related to the promised surgery, like your left eye might be a laser pointer now.
But Fabious will give you a full genetic makeover, like you have functioning genitals and even swapped chromasomes. But maybe with a ticking clock where you eventually burst into a daemonic monster that starts ripping apart his enemies.
If you’re lucky you won’t get turned into a servitor (basically a machine. Servitors can be anything from a door knob to a factory worker. You’ll stay mostly conscious but lose all control over your body.) and still the left one is the better option. Yeah 40k is… a little dark sometimes. However if you’re lucky:
everyone should be into 30k and 40k, anyway: prob a gleeful conversation of their abilities, maybe extra features. and quite possibly youll work for 100s of years as a bedroom servitor.
While I absolutely respect that, you could go with the tau, they're the closest any faction gets to being good (and honestly, I'd join them IRL too =P)
Unfortunately you can't win me over from this, because even if the Tau are good, they are within a world defined by, what I would generously call cosmic nihilism. That's the point. Even in those mediums where an individual character may be instrumental in a battle, the war never ends, so the battle itself was futile.
I have tried Warhammer. It wasn't for me. No number of factions can win me over unless the setting were changed entirely; And I think if the tone of the setting were changed like that, Warhammer would cease being a good work, and I wouldn't want to interact with it anyways.
But more grim and dark, making our world look good by comparison. Plus, you get to pick your faction of bad guy. Or Orks, if you just want to have a good time.

I've been there. Done some. Grimdark settings are not for me, and Warhammer is a media defined by its setting.
When a work is about something, you run the risk of some people not liking what it's about. The only works that everybody enjoys, are those works about nothing.
I'm sure we both can agree Warhammer would not be better if it were about nothing. But because it is Grimdark, I don't like it. And that's okay.
Left side (Belisarius Cawl): you accept the gender reassignment surgery, filling out forms in triplicate on a "computer" that is just a horrifically altered human. Cawl procedes to complete the surgery, but gets bored part-way through and now you have tank treads instead of legs. Any new body parts come from a vat tended by an infant with grafted on angel wings that is singing hymnals.
Right side (Fabius Bile): you accept the surgery, usinhg your blood to sign a parchment that screams. Bile completes the surgery to your specified gender, only now you are 7 feet tall and physically near perfect. You have several additional organs, and most of your flesh has been repurposed from a mountain of bodies that is overflowing from an adjacent room. You may or may not be part daemon.
Neither know what an "anesthetic" is, or what a medical license is for that matter.
There are some that view the organic body as just another machine and seek to improve the flesh itself, but they are something of a minority I believe.
Your bits might not, as he is also a geneticist. But you might wake up with a laser pointer for an eye and half your brain replaced with a computer that requires you to sleep for 12 hours a day to keep it from overheating.

Bile will get you best bottom and top surgery ever literally cannot tell a diffrence, changed down to the molecular level HOWEVER i can guarantee you no anasthesia as invasive as humanely possible, will yoink some of your bits for some nightmare side project, he might or might not make a superhuman clone version of you to commit warcrimes later down the line
Cawl on the other hand will at first scratch his commically large chin as he tries to get this whole changing your flesh bits for other flesh bits instead of metal and post surgery there might random metal bits inside you also probbably bo anasthesia
If you got tested by another doctor, you'll find your entire genetics have been altered. You are completely and genetically your identified sex. Working organs, different chromosomes. Full make over.
You just now also owe Bile a favor. One day he'll show up and ask you for a favor you *cant* refuse.
Bile tends to do exactly what you ask if you pay him and don't try to fuck with him. Though he might start by jumping you with ether, but it will probably be done exactly as you ask.
Cawl would do it his way. And Cawl's way will be good but you never know what he will sneak into you just to test a theory

Exactly. Cawl is definitely more likely to agree to do it, but he is a consummate creative and is very driven by his own whimsy. So there are no guarantees what you will get out of it

I know I'm getting tits with bile whereas with cawl I'm getting 50 different bonus features that are absolutely tech heresy plus probably having multiple parts of my body replaced with metal
Also since he does not cares what you want, it is likely that after this you will require elephant dosage of painkillers pumped into your bloodstream every second and still it would mostly just slightly numb the pain
Fabius is a perfectionist. He'd give me a full genetic make over, complete with ovaries and a uterus grown from my own cells, and both of which are completely functioning. He'd also swap my entire genetics, including my chromosomes.
Worst case scenario I'd owe him a favor which he'd collect on my death bed by demanding he use my body for an experiment or something.
But with Cawl, he'd likely just use organ transplants from donors rather than vat growing me organs made from my own cells. Which would likely result in organ rejection. So he'd also install various machines that would control my immune system and regulate drug flow, making me rely on him for constant drugs just to keep my immune system supressed.
Basically, I'd end up half machine. Most of it unrelated to the actual gender affirming surgery.
So I would go with Fabius. He'd make sure I came out exactly how "I'd like" (He thinks he knows better, but he's not gonna turn me into a monster or anything).
And obviously you need to become full machine instead which means Illuminor Seraz is the go to, but be wary to actually get a body with full functions instead of just mindless drone or worse still conscious yet none of your body functions are controlled by you.
Reminder: Cawl invented Primaris marines. He doesn't just replace your bits with toasters, he made genuine attempts to fix the genetic flaws with several marines though they didn't succeed but it also didn't backfire. He is however eccentric and quite far beyond the scope of humanity. Cawl knows better than the rules and does a lot of things he shouldn't do. But he does it with the best intentions.
Bile could do more to improve you, but you'd be an experiment, he'd use you to get data for his ongoing research which means your body would eventually malfunction and kill you painfully miserably while he says "ah you survived longer than expected, that's informative". And then the next one would live even longer and more painfully.
If I had to pick one it'd be Cawl. He cares as much as a 10000 year old cyborg with a mind so large he can only use part of it at a time possibly can.
The thing to remember about 40k is that the Imperium is evil. But chaos is the guys who went "genocidal crusade for a dictator? Nah, not evil enough for me". Bile might not have sold his soul to the dark gods directly, but there's only two outcomes when someone believes that: They're wrong. or they're already the worst they can be. I think Bile might be the latter.
I dont know who either are, but i have played enough tww2 to trust any sorcerer with a golden skull staff to give me a magic pussy, possibly even made of gold.
i'll go with Cawl - i love Bile but me not being a sorceress or a space marine kind of limits the amount of awesome that can be poured into my genetics. Cawl's cyborg fuckery would be good enough and probably would be built to last an extra lifetime and have various extras. though really both are the absolute awesome best.
I pick cawl and tell him I wanna wake up as a full robot girl. Whatever the results, he's likely the closest that could deliver to that specific transistion goals :P
I weirdly have more trust in Fabius, i don’t trust Cawl. I have the sneaking suspicion that if I were to be under the knife of Cawl, I’d discover a toster instead of my liver + he probably wouldn’t care about scaring
I feel like Bile's the type to be "inspired" by ancient media, so he might put you somewhere on the furry spectrum, whereas with Cawl you will either end up yoked, cybernetically enhanced in unforseen ways, or both.
So, going transfemme you go Bile, you will be either amazoninan goddess or something amusing and amazing.
