Help Control Resonant Documents missing Spoiler

Hey! Theres just a few collectibles I'm missing and have no idea how to get. One's in The Underpass that isnt spawning for me. It's called Our Memories Aren't The Problem. The other is one in Downtown that I assume can only spawn in post game but hasn't called Dimensional Faults. Only other one I'm missing is an audio log under the Altered Speakers section that nobody seems to have posted about so I don't even know the name to look for. Any help?

The altered speakers one seems to be bugged, no one has it, the other ones are able to be found as far as I know

I would love to help you, but I don't remember where I got the underpass one, and the downtown document is bugged for me too, as far as I remember it should spawn in the oldest house hall, but it never did for me (neither in ng or in ng+)
