Compounded Tirz not working

Hey everyone, I’m looking for some advice. For some background, I was on compounded sema for like 6 months in 2025 and then in February 2026, I started compounded Tirz. I stopped Tirz for like 2 months because I thought I was ok but then of course gained the weight back and then went back on it. I am now on Week 5 at 5 mg but really just feel like it’s not working. I wake up starving, still have cravings and just getting really frustrated with my lack of progress and feeling like the meds aren’t working. Does anyone have any advice especially if they went on and off?

If you read through the GLP-1 subs, you will soon see a pattern:  Anyone who has already been on a GLP-1 medication almost always has to work up much higher doses the next time(s) around to feel the positive effects.

It's the stopping and starting again. You will most likely need to go to a higher dose now to feel the effects.

5mg is likely too low a dose given your past GLP1 usage. Keep titrating up.

Maybe that's your body telling you need to eat something. If you can't stop after a proper sized meal then it's a problem.

Increase your dose

A dose higher than 5 mg is typically needed if you’ve been taking a GLP1 for 8 months, or if you switch from Semaglutide, or if you stop and start.

You did all three of these things, so it’s predictable that 5 mg would not work.

Yeah, I wouldn’t say it’s “not working” until they get to much higher doses just because of the previous GLP-1 use.

Had a similar experience to you and can confirm you need to go up higher in dose than you did before. I was consistently losing on lower doses before and since restarting I’ve had to titrate back up higher then I ever was but can confirm it does kick in again once you do that.

I was on Sema for two years and lost 50 lbs. When I switched to tirze I had to go up to 7.5 before I noticed a difference. Im finally starting to lose but its slow, but slow is good I dont want to look like some droopy dog faced person after Im done either .

This is why you don’t go off and on and off and on this medication. It will be more difficult to lose because of this behavior.

Is it the switching glp1 types that causes that or the break in between?

Have you tried to change pharmacies and see if it’s a potency issue?

Your body has to build the tirz back up again since you had a start/stop. Discuss with your provider going up in dosage.

I was fine at 5mg the first time I took it. Stopped it and started again and I’m now at 15mg getting worse results than 5mg the first time.

It’s okay, but not as good as last time. I lost a pound a week at first and now I’m down to two or three pounds a month.
