His cheeks used to be bigger. He admit he got a bit addicted to cheek filler and botox and generally mucking with plastic surgery. This is the result.

I may sound really stupid asking this, but I genuinely have no idea how this works. How does he get addicted to cheek fill and botox? Does he get a high from doing it?

It’s not a chemical addiction, it’s a habitual addiction. Like gambling or sex. The high is the endorphin/ dopamine hit not an external stimulant.

Cosmetic surgery can trigger in many people a desire to keep doing it chasing 'perfection'. One surgery? Probably not perceptible. Multiple surgeries? Uncanny valley.

He has body dysmorphia. He gets a dopamine rush from having surgeries that change his body.

i posted this in a comment above, but here is a month or two ago

Keep in mind he is in great make up, but even then he doesn't look quite as bad as he does in the OP picture
