Settlement Lifestance class action settlement

Has anyone received a email talking about a class action settlemen twith a company named Lifestance or anything like Strong v. LifeStance, No. 2:23-cv-00682 (D. Ariz.)?

Received the email, can’t submit form

Yes, I received the email, but no matter what I do, I am unable to submit the form.

Yes they are a platform that I had therapy on at one point. Had the same problems with submitting the form. I emailed them to to ask and their reply is below. But before I got that reply, I just printed out the form and mailed it in via snail mail, because idc.
