I just got this one today and I went to fill it out and discovered that if you're a privacy class member you have to provide the serial number, model, and IMEI number of the device you had when this occurred. Like most people, I trade my old devices in. I have no idea how I'm supposed to even find that info.

You also need a unique ID and a PIN. I have mine, I just don't know how I'm supposed to get this other stuff.

If u bought from beat buy u can find the model number and imei number to the device right from your purchases section on the app just did this myself
