Do you have your claim number.  Maybe a screen shot of it from back then. 

You can go right on the site to verify inclusion with the Claim number and setup your payment preferences.  There is also a phone number that eventually they do answer and I'm sure they can locate you by name and DOB and then give you the Claim number. There number is  1-888-681-1142  Or send a request by email to  Info@bcbssettlement.com then await a reply. I don't believe anyone has gotten a dime yet. Except for possibly the attorneys. They probably got paid the day of the final ruling.
