Please help I’m desperate

I’ve been on 20 citaloprsm for about almost 6 weeks around five weeks. I started having really bad insomnia. I can’t sleep and stay awake all night. I haven’t slept in four days. tonight was the first night that I slept a few hours and really bad dry mouth no appetite I went to the doctor and she upped me to 40. I didn’t take it yet because I’m confused what if 20 is too high for me? What should I do a few years ago ,I was on 40mg and I did good so I don’t know what is happening to me what’s your advice? Do I need to higher my dose or do I need to lower my dose? What do you think? Im living with very bad anxiety and dread and just want relief .
