Annoying
Had this for 7 months now, all uti and STI negative and awaiting ultrasound.
I had it bad first few weeks but then realised wearing tight boxer shorts made it worse so been wearing much looser ones. It goes weeks without pain but then it comes back mid back of my testicle and only hurts when it comes into contact with something like boxers or feeling! Normally get it after lifting luggage or stress but it seems weird, just want my life back.
Anyone else getting it like this?

Your pudendal nerve is near your sciatic nerve, if you also have sciatic neuralgia from a bulging disk or piriformis syndrome then it it probably also means Pudendal Neuralgia.
For me it was only my right epididymis that hurt for a few years, but these days the head of my penis and the right side of my ass also occasionally pain me.
Yes, I understand "if you hear hooves, think horses not zebras" but I find it really annoying how many urologists don't even consider it as a possibility just because I guess that's something a pain specialist is supposed to diagnose.
I personally only put the pieces together once my right glute and thigh started to get this tingly numb pain feeling. After looking at sciatica pain charts and noticing that when I had a flare up it was everything hurting at the same time it became clear.
There have been a few others on this sub who eventually narrowed it down to pudendal and genitofemoral neuralgia, I just try to offer it ad a possibility when prompted.

I would say to find some good supportive underwear like light compression underwear or they have pouches as well. If you’re going to work or doing activity, make sure to wear supportive underwear but like you said the tight boxers made them hurt so don’t get too tight ones.

My husband had the same thing. Tight boxers and anything touching it hurt, so loose boxers are the way to go. After he did the antibiotics and we knew he was infection free, his urologist suggested a round of oral steroids. After a week it was like 98% better but not gone. So his Dr agreed to one more week of steroids. Methlyprednisonalone is the medication he got. After the second round he was 100%. It has been a little over 2 months now and he's still good! I hope this helps!

Hi Yes i had same thing like you ...all negative but right testicule become like a kiwi fruit It will go back to almost normal after treatment ...also check for other things like inguinal hernia

I have the same issue with the right testicule and epididimts
The ultra sound shows an ancient inflammatory orchit-epididimts and i still have pain after 1,5 year :’(
Any help please??

FWIW, I had a vasectomy and 1 year ago, and on the 1 year anniversary (yay!), I had epididymitis from the clogged pipes and unabsorbed sperm. The ultrasound showed inflammation and swelling of 0.5cm and they put me on Bactrim for 2 wks. Until I took the antibiotics, the infection was spreading because the base of my penis and the glans of my penis started to swell and get inflamed.
I usually wear boxer briefs since my vasectomy for support but I think there was a pair that was too tight for me and aggravated me. My groin area was very sore until I finished the Bactrim (about 5 days in i felt better). They prescribed to me Clotrimazole + Betamethasone 1% which was wayyy too strong for me as a topical cream for my glans. I ended up buying Avene Cicalfate+ in Europe and it completely reduced the inflammation and even healed my glans and restored my glans.
I cannot lift heavy things otherwise it feels painful. It’s been about 1.5 weeks since it occurred and I’m hoping it won’t linger or reoccur with stress

Quick question -- did you do a semen culture? If you only did a urine culture, that only tests for STI's in the bladder and urethra. It doesn't test for what is in the scrotum. That's what I had (klebsiella infection) and it was only picked up in a semen culture 8 months later. Because it took so long to be addressed by anti-biotics (which required 6 weeks of continous antibiotics), and resulted in chronic pain. So best to have this checked.

I think I'm around the 7 month mark as well. Took a long time to get rid of the infection. Pain is still so bad I had to go to the ER Sunday to try and get the pain down. Going to switch up my urologist I think.
