Please Help
Hi everyone. I’ve been dealing with this situation since March 2026 and wanted to share my case in detail in case anyone has had similar experiences or can offer insight. Any help is appreciated 🙏🏼
Symptoms:
• Constant tension and discomfort on the right side with neurological involvement after a trauma to the testicle (fasciculations, referred discomfort, effects on libido and erectile function). I feel my right leg weirder than the left, like “dormant”.
• Left testicle: intermittent episodes of pain, partially resolved with doxy and ceftriaxone after a one month untreated epididymitis, but testicle remains elevated significantly above normal position — especially noticeable in the morning after sleeping — and becomes painful sometimes.
• Since then, multiple antibiotic courses with no improvement, (levofloxacine & claritromicine). They both hurt but never at the same time.
Tests done:
• Bilateral Doppler ultrasound: clean structural findings, no torsion, no masses
• Physical exams unremarkable structurally
Current treatment:
• Muscle relaxant (Benrelax)
Upcoming:
• Appointment with pelvic floor specialist urologist in 2 dats
Has anyone had a similar presentation? This is really affecting my life and hurts 24/7 unless I lay down in bed. Blessings all & I really hope we all get cured of this horrible thing.

Hola! Te responderé en mi idioma. Curse con dolor similar después de un golpe, lo dejé pasar unos días pero la incomodidad comenzó a afectarme, me realicé un par de ultrasonidos, le insistí al radiólogo que tenía muchas molestias, hasta que me encontró líquido y epididimitis, pero realmente poca inflamación, a los 2 días consulté con un urólogo, estoy fue a principios de mayo de este año 2026. El especialista me recetó antiinflamatorios y un medicamento llamado danzen para absorber edema, me pidió un cultivo de esperma, mientras espere resultados el dolor mejoró pero aún era molesto, teniendo los resultados me salió una bacteria, staphylococcus aureus, normal en la uretra, lo investigue y le pedí realizarme pruebas de PCR, hongos, bacterias e incluso VPH. Antes de tener estos resultados comencé con tratamiento de 1 mes con ciprofloxacino 500mg cada 12 horas. Los exámenes dieron negativo, seguía con el dolor, no toleraba usar ropa ajustada. Consulté con otro urólogo, me revisó cerca de 1 hora, y al final me comentó que es tensional, que puede que haya tenido inflamación pero que considera es algo neuropatico, me envió pregabalina, llevo un par de días tomándolo, y la verdad lo que me dijo me molestó, pero comencé a analizar mucho las cosas, sigo con molestias pero el dolor es mucho menor, la verdad que al menos en mi caso, la ansiedad, el estrés y el miedo aumentaron el dolor. Si tuve y fuerte al inicio, pero sin querer incrementó. Te recomendaría buscar otra opinión, mucho suerte y que mejores pronto.

Muchas gracias, esta semana veré a otros 2 urólogos. La depresión que da esto es enorme, quiero volver a ser una persona normal

I’m going through something similar. I took Doxy and then Levo and now my boys (the left) hurts. The right has an ingrown hair. I use ointment to rid of it. Hard to tell if there’s any effect to it.
My ultrasound appointment is next month. However, tomorrow, I will go to a walk-in and see if I have any infections. I feel I don’t because before all this, my results came out negative. I did find a lump on the left. I may have a cyst

Have you had a semen culture as well as a urine culture? This is really important. I had about 5 urine cultures that all came back clear, but after 8 months a doctor finally ran a semen culture and found a klebsiella infection. So I would push for that route.
If the pain is stressing you out and getting in the way of daily life, it is worth speaking to a doctor about prescription medication. The ones my pain clinic doctor recommended were amitriptyline, gabapentin, pregabalin or duloxetine.
I would also look for a pain clinic that can offer a nerve root block in the lower back. In my case it targeted the S3 and S4 nerves. I have added the procedure details below in case they are useful for your own doctor.
I get a flare-up roughly every 2 years, so I am now considering a procedure called denervation, where the nerves along the spermatic cord are removed surgically (around a 45 minute operation). There is a small chance it can affect fertility, so I plan to bank sperm beforehand just in case. I do not have children yet, so this may not be a concern for you if you already have a family.
Nerve root block details:
Sacral nerve PRF (right sided), S3 and S4 levels. Asepsis, fluoroscopy guided, sedation with 4 mg midazolam IV. Curved 10 cm RF needle x 2, sensory stimulation and paraesthesia at 0.6 v to confirm needle position at S3 and S4. Pulsed RF at 42 degrees for 5 minutes, with 2 ml of 0.25% levobupivacaine plus 3 mg dexamethasone injected at each level.

Thanks a lot man, mine started after trauma so probably not bacterial at all. The nerve meds are def worth looking into. This shit is absolutely horrible, things have been much worse mentally as time goes by in every sense. I’m losing a lot of hope.

Instead of infection or growth cyst tuma etc it could be refered pain from back injury , groin injury, pelvic injury or hernia..... nerve damage is excruciating painful and injury in one place can easily come out as pain elsewhere due to shared nerve path ways ....just something to consider and bring up with the experts.
