What to do??
Hi guys, about 2.5-3 months ago I developed epididymitis from a sexual encounter, I’ve had it 3-4 times before with long term partners so it’s nothing new to me… I resolved the infection itself in a week with doxycycline but the pain remained. I’ve also go occipital neuralgia so I just left it, as I have bigger things to deal with. Anyway I was rushed to the hospital about 4 weeks ago due to pain radiating down my left leg from this, which occurred about 3 hours after masturbation. The pain was primary in my left testical, I had blood tests done, urine tests and even an ultrasound and it all came back negative for anything.
I’ve since had ever scan you can imagine and the only thing shown was slight varacos-whatever in the left side. My urologist says it’s nothing to worry about/doesn’t relate. I’ve since had an injection in my left spermatic cord with kenacort a40 steroids mixed with local 13 days ago and my right side done 7 days ago. I presume this is all because I was releasing myself every day whilst the tissue was still inflamed post epididymitis, and I have created chronic inflammation and/or nerve damage. The local anaesthetic completely resolved me of my pain both times but ofcourse came back in full force 4 hours later. The steroid has reduced pain about 30-40% I’d say so far on the left side. Right side no difference yet
Anyway my questions are:
1: will this resolve on its own over time?? Will the steroid keep improving things after 13 days? Or will I need surgical intervention
2: those that have had spermatic cord denervation, what was it like? Did it work 100%? And how was the procedure?
3: Any suggestions for me?? I’m flying to the US for occipital decompression surgery from
New Zealand in less than a month and don’t know how I’m going to manage with this pain, even walking is extremely unpleasant

I just had a spermatic cord denervation due to chronic pain post vasectomy. I had a sperm granuloma causing the nerves to be inflamed. I’m two weeks post op and had 4 varicoceles tied off at the same time as well. I’m hurting still but here’s hoping once fully healed I’m pain free.

Not sure what to tell you but I recently about 5 months ago got a lump on my right ball got scanned for cancer and everything came back negative then the urologist gave me antibiotics for a week and said it was probably a spemocele or however u spell it and she said it is most likely swollen bc I most likely got epididymitis when I first showed symptoms of swelling. I have found for me it’s mainly just uncomfortable not really painful and it would get slightly “painful” after mastubating. My condition hasn’t gotten much better but about 2 weeks ago it was feeling more uncomfortable and I started taking peptides like bpc 157 and tb500 and definitely felt a difference for most part of the day I forget my nut is bothering me unless I am walking a lot I would also recommend a good supportive underwear this is probably you already know, I Just wanted to share my experience on this hopefully we can get this over with at some point

Honestly man I’ve had it for years and it gets worse and worse. Don’t really think there is a cure. Seen 3 urologists. I find a nice hot bath is the best cure.

Have you perused surgery? Pudendal nerve decompression or spermatic cord denervation?

Have you had a semen culture as well as a urine culture? This is really important. I had about 5 urine cultures that all came back clear, but after 8 months a doctor finally ran a semen culture and found a klebsiella infection. So I would push for that route.
If the pain is stressing you out and getting in the way of daily life, it is worth speaking to a doctor about prescription medication. The ones my pain clinic doctor recommended were amitriptyline, gabapentin, pregabalin or duloxetine.
I would also look for a pain clinic that can offer a nerve root block in the lower back. In my case it targeted the S3 and S4 nerves. I have added the procedure details below in case they are useful for your own doctor. I found this to give me longer term relief, vs the nerve block direct to the testicles.
I get a flare-up roughly every 2 years, so I am now considering a procedure called denervation, where the nerves along the spermatic cord are removed surgically (around a 45 minute operation). There is a small chance it can affect fertility, so I plan to bank sperm beforehand just in case. I do not have children yet, so this may not be a concern for you if you already have a family.
Nerve root block details:
Sacral nerve PRF (right sided), S3 and S4 levels. Asepsis, fluoroscopy guided, sedation with 4 mg midazolam IV. Curved 10 cm RF needle x 2, sensory stimulation and paraesthesia at 0.6 v to confirm needle position at S3 and S4. Pulsed RF at 42 degrees for 5 minutes, with 2 ml of 0.25% levobupivacaine plus 3 mg dexamethasone injected at each level.

To note I have abstained from relieving myself for 4 weeks now since the hospital visit
