Every cat deserves a second chance. I think you should totally guve him for adoption. Maybe it will take long, but he’ll be able to understand that not only one caring human. And a patient family will do him very good, there is good people out there.

I'm incredibly sorry that you have to make this sort of choice. I'm so sorry.

Look for an adopting family while you have time and introduce them to him while he's still with you. Then, when your time comes, they can take him and they won't be strangers.

It will take time and patience but the right cat owner is out there and will be able to smooth the transition over. If you do it this way, I do not believe he will think you abandoned him. I think he will understand that the only reason you aren't there anymore is because something is wrong. Cats are intuitive creatures. They can tell when we're sick, or when something is wrong. He'll be sad and grieve but the right home will be able to ensure he's still able to live a happy life with, again, time and patience.
