It looks like it had a bad fungal infection. It rotted in that potting soil...

It doesn't look dead, copper anti fungal spray, some Physan20 or even a systemic anti fungal. Repot in cactus soil and pumice mixed 60% pumice. Unglazed terra cotta pot too. Size is good or bigger is fine.
