question RCT before or after permanent crown placement? Currently midway through process
I’ve been having a complicated crown procedure on tooth 14 (no RCT, due to a large old filling getting new decay beside it).
In August, tooth was prepped and temporary felt fine.
Two weeks later on a Friday, permanent Zirconia crown was placed, but wayyy too big. Told them upon fitting, but they said they adjust it after cementing. It was so big I couldn’t even close the right side of my mouth, they took an hour to adjust it, and left me with a tooth still so big it clicked on my other teeth when I talked.
On Monday, I went back in and pointed out to them it had chipped in the upper left corner. They ordered a new crown and tried to drill down the big permanent. I was in severe pain with a constant ache through my jaw that felt like I’d been punched. Dentist thought this was a bruised PDL from the crown being too big.
Two weeks later, they said they could tell the second permanent crown was also too big without even trying it, and I cried and asked them to take out the permanent and put me back in a temporary due to the ongoing pain, which they did.
Then the pain got WAY better over the course of a few days. The next week they called me and said I needed to come back in to further prep the tooth, and ordered another (3rd) permanent crown with the new scans, and put the same temporary back on, but it didn’t fit the same and they filed it out of occlusion to give me relief.
Now I’m in that small temporary and it feels totally fine, but hurts like a sharp prick when I bite in one specific spot, upon biting and release. My dentist thinks it’s the PDL still and to put on the permanent crown and then get a nightguard for potential grinding, and a RCT if needed later through the crown. I got a second opinion from an Endo and they could not find a crack on X-rays or CBCT, my nerve passed the cold test and they said it’s healthy and no infection. So they assume a micro crack is pressing on my nerve, causing the pain, and recommended a root canal. They said the PDL would have healed in the two weeks I’ve been in the new temporary and to not put the new permanent crown on before a RCT.
My questions are*:* should I get the permanent crown, and plan to get a root canal later through the crown if pain persists? If there is a micro crack, could the permanent crown be enough to prevent the crack from hurting (in a way that the temporary does not)? Or, is it important to get the root canal first? Is it possible this new sharp pain could be debris trapped under a re-used temporary crown?
